I began CPAP in October 2007 - initially using an Activa - which although large and cumbersome, provides me with low leak rates and decent average nightly numbers (AHI averages around 3). I picked up an Opus 360 nasal pillows mask a week ago - looking for something a little lighter that accomodated my floppy, occasionally face down sleeping habits a bit better. I've used it 5 out of 7 days now, and while I love the Opus 360, the numbers have been disappointing.
The leak rate shows quite a bit higher (around .3 on my Resmed S8). My AHI has been between 7 and 9 (average in the high 7s). I don't feel any leaks, my wife doesn't hear any obvious leaks. I haven't woke up to any leaks that I'm aware of. I'm also under the impression that the machine should compensate for any leaks it detects.
The higher AHI is also puzzling. Is it time to run up the white flag and admit this mask isn't working for me or has anyone experienced an "adjustment period" with a new mask? I'd love to keep it. It seemed to fit right out of the box. It's also very comfy and lightweight . I don't have a real problem going back to the Activa (aka the rubber starfish) - it's large, a bit bulky, and certainly doesn't make a fashion statement - but I got used to it.
If it matters: ResMed S8 Elite, currently set at 10 (prescribed 9), EPR off, new 6 foot hose (the numbers are fine using the Activa with this hose - the CPAP hose setting is correct) and I chose "Standard " for the mask type since Opus wasn't a choice.

The older I get, the better I was
A leak of .3 with the mask set at Standard should be OK - but look for graph in the mask's manual. Resmed subtracts the intentional leak rate from leak reports for masks it knows - but doesn't do so for "Standard" (i.e. "I don't know you from Adam") masks.
