Any one Else ?

I would be interested to find out if anyone else is having the same problem as I am and how they are dealing with it.

I have been under treatment for OSA for about 10 years now and am 100% compliant with CPAP with a pressure of 10cm. Sleep studies have shown that my apneas are controlled by the CPAP but I am still extremely tired during the day.

A bit of history. I was first diagnosed about 10 yrs ago and opted for UPPP surgery (uvulopalatopharynogoplasty) because after my first experience with CPAP in my sleep study I didn't think I could deal with it every night. Well, what they tell you before the surgery and what happens is like night and day. Their description of the surgery "It will be like the worst sore throat you have had." Actual experience, weeks of extreme pain, months to feel better in the throat and over a year before my throat felt normal. The good part was a 35 lb weight loss because I couldn't eat. The surgery (or maybe the weight loss or maybe both) worked for about a year. 1 1/2 years later I was back to exhaustion and depression due to poor sleep. Another sleep study and I started CPAP. I did well for a couple of years but exhaustion returned and after a pressure increase I then tried Bio-Feedback therapy (waste of time and money). I switched Sleep Dr after my Doc basically said he had tried everything and couldn't help me beyond what he had done!
The new Dr was/is great. She increased the pressure and that seemed to help for a while but once again the tiredness returned. Last year I participated in a 4 month study for a drug ( R-Modafinil (NUVIGIL) ) to help people with OSA who are controlled with CPAP but are still tired. The study consisted of two sleep studies and 6 full day hospital visits and either the drug or placebo. Well after the study was over I elected to participate in the follow-up study where you would get the drug for one year. As soon as I started taking the drug the side effects started (so I am sure I was getting the placebo during the initial study). The side effects were greater than I could take and I removed myself from the study. My normal sleep DR then prescribed the version of Modafinil (Provigil) that is approved and on the market but that too had side effects that I could not tolerate. I decided to try weight loss to help and 67 lbs later (since Jan 1) I still feel tired. I am currently taking Ritalin (yes Ritalin - it’s a central nervous system stimulant) to stay awake during the day. I helps keep me awake but I don't like the way I feel with it and I am still tired and feel terrible.
Anyone in the same boat or have any suggestions?

Since it sounds like your apnea is being treated, have you had your thyroid checked?

A slow thyroid can cause extreme daytime fatigue as well as many other problems.
I would suggest having it tested if you haven't already.

Did your sleep studies look for things other than sleep apnea? You may have another sleep disorder. For example, although I improved a lot with Xpap, I am still tired after having only been up a couple of hours in the morning. I finally had another sleep study about a month ago and found out that I have severe PLMS (Periodic Limb Movement in Sleep) with a PLMS index of 75 (50 is considered severe). When I had my original sleep study it my PLMS was 76, essentially no change. My pulmonologist told me then that PLMS offen disappears with xpap treatment and to just wait and see. Now after one and a half years of being on xpap I was retested, and found that indeed I did also haved PLMS. I just started Requip to see if it helps but its takes a few weeks to see the effect.

My point is that maybe you also have an additional sleep order which has not yet been diagnosed, and that why you are still tired.

this is going off thread for a bit. My husband suffers greatly from restless leg syndrome due to iron mal absorption anemia. He is merely tenths of a point away from a chemo-like iron IV treatment. But my point is, have you had a good blood screening, hemoglobin. One big sign of iron def. is restless leg syndrome, which may not be related to the limb situation you are experiencing, but may just be the little silent tweaker no one has tested you for, during all your hospital stays/tests/etc.

Might be a thought..Also thryoid.
Can't tell if you're a man/woman, but many many men suffer from thyroid hypo/hyper prob's and doc's as we WELL KNOW, tend to negate that this is something to be checked out, and NOT just a normal TSH, a full thyroid panel, T4/T3, uptake, the whole 9 yards. Hypo thy would make you have the day time drowsies. I know, it does me.

Not a dr. just a patient, with let's just say experiences!

You know, almost everybody I know with SA has much improvement on CPAP but also still feel fatiged and tired. I wake up slow and fade some in the afternoon. But I used to all the time so it is really better. Some blame it on sleep deprevation and having to make up for all the lost sleep but I also contend that we still don't get normal sleep like people with no sleep issues. I drink lots of coffee and tea and it helps. I happen to drive a good deal in my work and just can't let myself fall asleep. If all else fails, I playu rap music which I detest
