Advice on narrowing APAP range based on oscar graph

Post by Zurindas » Fri Dec 19, 2025 11:01 am

It's my first time posting and I tried to read through some of the new user info but please let me know if I'm doing anything wrong!

I've been having some issues with daytime fatigue (I pretty frequently nap) and waking up generally groggy and not feeling super rested. I think there are definitely some other factors but from doing some reading I realized that the APAP range that had been set for me about 3 years ago had never been changed or really looked at and I was a little concerned that it should be a bit tighter. The range I had been on from the start was 5.0-15.0 and my AHI is very low, I get around 0.6 most nights and on the worse nights it will spike to about 1.5 at the absolute most (maybe once every week or two) so maybe I should be less worried about my settings but I pretty frequently get awakenings in the middle of the night or very early in the morning which I feel may be causing me to get less restful sleep and I am worried may be caused by the machine spiking to very high and then very low pressures.

Ive had the pressure at 7.0-11.0 for the last 2-ish weeks and its been okay but as you can see in the first graph I sometimes see it going high and flatlining at the top which makes me think that higher pressure could be potentially beneficial. My reasoning for 7.0 minimum is that even though my median pressure up until now sits around 5.8 my understanding is that my lower end should be a bit higher so the machine doesn't take as long to get up to the higher pressures when an event happens. I've seen some other people's graphs and they look a lot less spiky than mine. My main concern is that my lower end is not in a good spot, I feel like it should maybe be a bit higher so that theres not as much of a difference when the pressure is raised and hopefully causes fewer disturbances but I'm not sure if that's reasonable justification for raising the pressure any more on the lower end.

As for the higher end it seems like the pressures above 12-ish (based off the Dec 18th graph) tend to cause leaks and I assume it's because the mask is getting blown off to some extent but I feel like it's fastened very securely. In essence I'm wondering if anybody could give me advice on whether my lower end needs raising to reduce the large spikes and if my upper end needs to be adjusted to make sure all the events are being properly controlled.

For the two nights you posted, the number of events is near zero.

Your pressure line is relatively stable. Which position(s) do you sleep in?

You can always raise the max pressure and see what happens. The machine will not take the pressure any higher than your airway feedback says it needs to be.

SleepHQ is easier to use and allows us to zoom in on the detail.
