Need help - past month felt exhausted (Oscar screens included!)

Have been using my CPAP for a while now, and while I felt the impact immediately, the past month or two feels like I've totally fallen off a cliff - waking up tired, needing to grab a coffee or energy drink in the afternoon, etc. Not totally crashing like I used to, but definitely feels like a dramatic difference than before.

Initially had a 4.0 to 20 range, which I think is the generic range, but gradually adjusted the lower limit to 7.4 via trial and error - but now not seeing at all the same difference as I was initially. Haven't had a problem taking my mask off at night or anything like that, it feels like I wake up with everything still in place. Hoping y'all CPAP experts can take a look at my Oscar reports and offer some advice to help out!

Resmed AirSense 10 with P10 nasal pillows, range set from 7.4 to 20

(Note I only started using the SD card about 1 month ago and that the gaps are usually while I was traveling - I use an AirMini with P10 nasal pillows and same settings as on the AirSense)

Guess I'm at a loss for how to get more! I was asleep around midnight, I use an Oura ring for sleep tracking and it's telling me I slept for 7 hours that night (vs the Oscar report). In the report, there's a huge block missing from 2:15am to 5am - assuming something happened with my mask that night. But here's some other nights from that same timeframe in Oscar.

Do you remember waking very often during whatever hours you slept?

Do you take any medications of any kind? If so, what?

Have you had any change in medications or dosage or general health that could maybe explain the change in fatigue levels?

Any new diagnosis????

Your reports don't stand up screaming "fix me and you will feel better".....wish they did because that's usually fairly easy to fix.

I usually wake up once during the night to use the restroom, but that's about it - fall asleep pretty quickly once I lay down (occasionally end up taking 30-ish minutes to fall asleep, but not often), wake up to an alarm and don't have trouble getting up.

I do - I started taking accutane mid-October, and I have been on a semaglutide for bit (dosage upped around beginning of October as well). Those are the only 2 ongoing medications I've been taking, also take a multi-vitamin, allergy medicine, and vitamin D every day.

No new diagnosis and haven't done a sleep test since the initial one.

You might look at the semaglutide side effects.....fatigue is a well known common side effect.

From a quick look I didn't see much that we might blame on the Accutane but I just did a quick look.

Yeah, this feels like it's probably most likely if nothing on the charts throws any red flags - I've been on it for a bit and only recently started feeling like this, but the timeline does roughly match up from upping the dosage to when I started feeling more tired.

If you are sleeping soundly for the most part and the report details aren't very exciting and you are getting adequate hours of sleep.....medication side effects or other health issues is most likely going to be the cause of increased fatigue at this stage of your therapy.

Unwanted medication side effects are a lot more common than people want to think and people tend to want to blame their sleep apnea therapy instead of looking at other potential culprits.

Example of my own.....I have really bad arthritis so lots of various pains but the pain meds actually cause insomnia for me.
Let's see....I can either not sleep well because of the pain or the medications side effects...LOL damned if I do and damned if I don't.

Yeah that all makes sense and it's not even something I would've thought about until you asked about it! Appreciate that, figure it is what it is for the most part right now until hopefully I go down on dosage and get back to a better state with it all.
