Put up a 60 second flow shot of the moment the phenomenon starts.

I don't think it's cardiac oscillations cause they're too big.

If you trace out the known inspirations (large waves above the line) it looks like 15 non-flow-limited regular breaths.

Since she's on biPAP, the rest of the junk could be machine chatter, especially because the expiratory limbs are so deep (i.e., you now have 2 breaths to exhale).

Here's the first two minutes of when it gets starting. As you can see I think it does start with these cardio oscillations. They do get 'big' but I think maybe they just become more pronounced and it really is just my heart being picked up on the RR? At least I hope so, and I'm not actually breathing like this because it looks like you elegantly put it, 'out of its fing mind' lol.
Since she's on biPAP,
I'm a dude by the way, haha. But no biggie! Hard to tell on here. I will say that this is from last night was on my Autoset 10, with an EPR of 3 but I've had it happen on my VAUTO as well. It really makes no difference. I could try my Vauto again and increasing PS maybe?

I was told that it's just a name and can be used by men as well. It has some pretty flowers on it though.

It was also the only option available covered by Canadian insurance.

Those small guys could be cardiac oscillations, especially since they seem to match what your heart rate should be, but I think those are different from the big stuff.

I love it....I am going to steal it....I have had lots of time where I needed that to express my thoughts in a more PC way.

I am going to step back now in this thread and just watch and learn....we are beyond my comfort level at this point.
I am doing good to maybe recognize the echo thing...beyond that and I am lost.

I think the way to look at it is cardiac oscillations, if present, will be superimposed on the respiratory waveform, not wipe it out. They're also very subtle, so may only appear on the expiratory limb and/or end-expiration. See if they're regularly spaced and what the rate is. If they match patient heart rate, then you have a good case.

But how do we measure real heart rate (when asleep) without a device that actually does that? Like a pulse oximeter.
The FitBit I used to have would give sort of a generic heart rate but I couldn't ever figure out how to target a specific timeline for heart rate measurement.

I think for our purposes, just get a resting heart rate around sleep time would be fine. In the above I'm assuming an HR of ~72, so that would fit.

OTOH, if his "resting" HR is ~120...

Mayhaps I shall try to find that image where I superimposed an ECG on cardiac oscillations. Every now and then some clear examples show up.

Sounds good. Take a generic heart rate number that is an overall average.

BUT...your comment about 120 rate made me chuckle... 3 years ago I made a visit to Las Vegas at Christmas (pre covid of course) and the marijuana was now legal there and it wasn't here in the Bible belt where I reside....so I planned to try some wacky weed to see if it helped my pain/sleep issues. I discovered that the smallest dose actually did double my heart rate (had the FitBit at that time) and it was running 120 to 140 at rest at bedtime. Not a good feeling at all.
It stayed elevated for a good bit of the night as well.
Needless to say....that experiment was a failure. I wouldn't make a good drug addict.
At the time I didn't think to check the flow rate or statistic numbers to see what might have shown up with that heart rate.
