New machine causing issues
Hi.
So I've been using a dreamstation for about 5 years now and for the last 4 of those things have been near perfect. By that I mean I sleep 7-8 hours every night with an ahi that is 90 percent of the time below 1. I use a dreamwear nasal mask after teaching myself to stop mouth breathing about 3 and a half years ago so all good.
Of course in the last week or so things have changed with the recall, I didn't mess around and was lucky to be in a position where I could purchase a new resmed airsense 10 as a replacement. It arrived and felt quite premium so I excitedly set it up.......well 4 nights in and things are not going well. My ahi is still very good at below 1 but.... I am in large leak territory for a chunk of the night, around 15% of the time according to Oscar and I'm almost certain it's because I'm mouth breathing. My wife says she can hear me and I'm waking several times in the night to a very dry mouth. I can't see anything particularly standing out on Oscar to explain it and certainly has to be down to be new machine as it would be an extraordinary coincidence otherwise, pressure is set between 11 and 13.5, exactly as on dreamstation. I have the EPR set to off and to be honest can't remember if I had the Philips version of EPR (AFLEX??) set to on or not.
Would appreciate any suggestions/advice as in just 4 nights I can already notice difference between how I feel when I wake each morning and through the day. Thanks for reading my ramble.

Can you provide a recent detailed report from OSCAR for a DreamStation night and a detailed report from a ResMed night please?
It's impossible to do anything but guess as to what might be going on.
Also...exact details if at all possible...like are you waking often during the night now and you didn't before???
Gotta figure out what might have changed with the machine change.
If it is just more leaks and the leaks are disturbing sleep then we have to try to figure out if the ResMed is maybe going higher with the pressure or something.

With regards the other questions, i would normally wake 3 to 4 times a night very briefly and quickly be back asleep. Don't think that's particularly increased these last 4 nights and as mentioned only difference being that in those awakenings now I definitely feel a dry mouth which wasn't happening before. Something is happening as I feel dreadful last few days after feeling brilliant for years. Even on very odd occasion I'd have by my standards a very high ahi (3 for me would be very high) I'd feel brilliant compared to the 4 nights I've had on the Airsense. Thanks.

I don't want to hijack this thread with my own Oscar charts but I'm interested in this topic also. I've switched from Respironics to Resmed and I am getting the same Large Leak flagging. More leaks flagged with Resmed than Respironics. I know the Oscar reports are more informative than the Resmed Happy Face but except for one night where I got the Sad Face I am getting the Happy Face on the sleep reports.

Turn EPR on to full time and set it to 2 or 3 and see how things go.
The leaks increased for some reason but then so did the pressure variations themselves.
You were using some exhale relief on the Respironics machine and none now....looks like about 3 cm relief so try 2 or 3 cm relief on the ResMed.
While your AHI is low and while the leaks are a bit ugly...not so bad as to really make us think the machine was missing flagging events but very definitely bad enough to cause wake ups or disturb sleep (you may or may not remember it) which then of course affects how we feel.
Let's try stabilizing the pressure a bit by adding EPR to sort of match what you were using and see if leaks also stabilize.
Without exhale relief you are using more pressure all night long...your body is used to exhale relief and it might just be rebelling and missing it. The brain likes predictability and you changed its routine.
Now I will say this for people about leaks....Respironics large leak territory is a lot more forgiving than ResMed's line in the sand at 24 L/min. I happen to think that 24 L/min is a bit ultra conservative in terms of sensing and recording and not necessarily in the toilet at 30 L/min

Thanks a lot for that. I'll try EPR at 2 tonight which still leaves me somewhere to go if there is still a slight issue. Makes sense to me. I ummed and aaahed about turning EPR on and seems like it might have been the wrong call. Will update tomorrow on how I got on. Many thanks.
