Do I actually need cpap?

My AHI is 38.5, but my lowest spo2 level is 91%. My doctor prescribed me auto cpap with 10.5 cm H2O pressure. I have been diagnosed with severe hypothyroidism. Do I actually need cpap? Or just treating my thyroid problem will solve my sleeping problem?

And SPO2 alone does not mean you don't have OSA or need Cpap.

There is more to sleep disrupted by apnea than just O2 dropping. Apnea causes arousals, which means your brain goes to lighter sleep stages so that you breathe. So you are not getting sufficient deep stage restorative sleep or REM sleep to keep your brain and body healthy and functioning well.

It will be difficult to get your thyroid issue under control unless your sleep apnea is treated successfully. Sleep apnea has deleterious effects on hormone production.

Since your AHI is 38.5, it would be equivalent to
somebody holding a pillow over your face about every minute and a half.
Like being water-boarded in your sleep.

You need CPAP whether it improves your hypothyroidism or not!

You oxygen is 91%, which is the lower end of the normal range (as medical science currently stands).

However, your sleep is very fragmented, which means that you can't fall into deep enough sleep, and that your total sleep time is diminished by these micro-awakenings.

Sometimes my breathing stops during sleep specially when i sleep on my back.

I am not taking any medicine that might interfere with sleeping.
