Obstructives at 20 pressure?

Post by Mazus » Wed May 26, 2021 7:39 am

Hi, I have a Resmed Airsense 10 Autoset, and it seems that my obstructives are persisting despite reaching the machine's maximum, what is going on? Is this one of those postural things? Last night's sleep is attached.

It could be simply positional...like maybe you are on your back or you have got in a position where the chin is tucked quite a bit which alters the airway position.
It could be REM sleep related though I doubt this as being primary since the pattern of hitting 20 cm isn't corresponding to when a person normally will have REM.

It's a bit difficult to really evaluate your graphs because you have them so tiny....you have included too many graphs which forces all the graphs to be so small it's hard to evaluate.
See this thread for graph formatting.

Options available....
Increase the minimum pressure to maybe 15 and see what happens.
Reduce EPR and see what happens.
Try wearing a cervical collar to rule out or in....chin tuck being a factor.
If you know you sleep on your back...try staying off your back as much as possible.

Finally...there's always the bilevel machine that can go to 25 cm.

It could be. My pressure runs 10 -12 with a foam cervical collar and 18 - 20 without a collar.

It's a cheap experiment. Collars run $7 to $10.

The maximum pressure you're getting to is 17 between breaths, and that's where apneas happen, you can try reducing the EPR, and raising the minimum pressure a couple of points, and see if you can prevent the apneas from happening in the first place.
