Opinions on MMA?
I'm carefully considering MMA surgery. I don't think I can CPAP for my rest of my life because it will increase intra abdomen pressure that will cause varicocle to come back. I had embolizaition for varicocle.
How successful is MMA? I'm willing to take risks if it can get rid of obstructive sleep apnea (16.6 AHI). I have a small bottom jaw. Has anyone gotten the surgery? How did it go?

Hmmmm....this is a cpap help forum. I doubt many people who might have had MMA surgery and have it be successful would be hanging around here....I know I sure as hell wouldn't.
You probably need to find a MMA forum. Doubt you will get much here except failures that are still on cpap or opted not to do the surgery.

Cpap is the gold standard--and most likely to be covered by insurance.

Back in the late 70's had jaw surgery. Wasn't called mma back then ( as far as I know but I was just a teen) They cut my jaw brought it forward and put in pins. It was a miserable / awful experience. 30+ years later, I have sleep apnea, and jaw pain every time the weather changes. It didn't prevent my sleep apnea it didn't prevent all the other dental issues they said it would, I wouldn't recommend it not even to my worst enemy. Just saying. They used my xrays and teeth molds to sell/show off how successful it was, they dragged me in for every dog and pony show the orthodontist had. I hated every second of it and finally was old enough to refuse to be used as a selling point for their butchery ! I know its not what you want to hear but don't do it. If I remember correctly, you are in your twenties. This would be a good time to make the decision. At this young age, you will heal from the surgery more quickly.
Yes, very carefully is the right way to consider this surgery. Read all you can that has been published about MMA surgery in the major medical journals. Here is one example - https://www.mayoclinic.org/medical-prof ... c-20430404
Every doctor that gives you an opinion should give you an estimate of your surgery's success.
Don't worry much about the antis in this forum. We are CPAPers. Few of us are as young as you. If I were in my 20s or 30s today, I would seriously consider MMA surgery.

One proponent claim the surgery provides up to 15 mm more advancement of the jaw resulting in bigger airway than "plain" MMA.
MMA's definition of success is not AHI below 5 but some percentage of reduction in AHI that varies between studies. So after surgery you might still need CPAP.
Another thing to consider, these surgeries only deal with OSA.
Even successful, you might have or develop centrals/CSR/complex apnea in the future.
Then your only option is some type of CPAP with backup rate (ASV/ST).

I'm 23 and I really can't see myself wearing CPAP for my rest of life.

But's that unlikely anyhow... med science moves fast and anything's possible in the near future. It's still better to do what's best now to be ready for whatever comes tomorrow.

What percent of your apnea is Central versus obstructive? Did your doctor tell you your trachea has a normal diameter or is it narrowed? Before you ever indulge in such surgery you need to make sure that the surgery will be successful and that any obstruction will be reduced. If you have a narrow trachea UARS and/or central apnea you are not going to have any relief from that surgery.
Weight loss, appliances, and anything else possible should be done short of surgery. Think of surgery as a last resort.
