Happy to report my AHI has decreased significantly. Was seeing 8-9 the last few weeks, but has dropped to around 2-3. I'm usually a nose breather so I use a nasal mask, but it turns out my turbinates have been swelling frequently causing an upper airway obstruction. Not something the ENT doc picked up on since they're usually fine during the day. Got some otc meds to reduce the swelling and seems to be working well with combined with the cpap.

Pugsy you were definitely right about poor sleep quality. I believe years of untreated osa has wrecked it causing moderate insomnia. Even with the AHI significantly reduced, I still feel like death the next day. I remember waking up quite often throughout the night, and the pulse oximeter shows frequent motion as well (every 20 mins or so). Previously I would be in bed for 9-10 hours, but only appearing to get maybe 5-6 hours of sleep, so I'm attempting sleep restriction training to mitigate that.

Anyone else that go through similar steps? Curious if sleep restriction worked for you, and reduced your tossing and turning at night and improved sleep quality combined with xpap therapy.

AHI and sleep quality have definitely improved. Fighting a bit of aerophagia though. Considering reducing the max to 11 since 95% of the time it's under that, and either slowing reducing the minimum pressure down to 6.4 from 7.4, or increasing EPR to 3 from 2. Not sure what route I should take. Any suggestions? Screenshots from the last 3 nights.

you are actually looking pretty good by my lights.

do you remember what settings you were at when you didn't have aerophagia?

if so, set the machine there and increase the increments by .2. let it settle in. if it flairs up, set it back to your starting point. let that settle. then try increase of .2.

this will take a while and some patience. it's what dog slobber has dubbed "the zonker protocol" as it was my method to get past my own aerophagia.

Unfortunately I don't remember those settings. I ramped up the pressure very slowly, so it was a subtle onset. Just gotten to the point it wakes me up and I have to clear out some burps and what not. Disrupts sleep more than anything else. Wife doesn't have any appreciation for it either. Really nice to have medical excuse for my farts now though. She can't get mad :lol:

But I could take a guess at the pressures and try the zonker protocol. Worth a shot. Thank you!

Just read the whole thread and it's inspiring how you moved from horrible results with the Dreamstation Go to these great looking ones. Well done!

Also, the algorithm of the dreamstation go with all those pressure spikes look really uncomfortable when comparing to the mostly smooth gradual changes of the Airsense 10.
Machine: Airsense 10 Autoset (humidity 7, slimline hose) (5 years)

Agreed! I think those spikes were waking me up constantly. Maybe if I were more used to pap therapy it would have been fine, but didn't work too well for my situation. Also find the EPR on the resmed much more comfortable than the flex on the PR. Very happy with the Airsense 10.
