You are showing a lot of segmented sleep sessions. Why?

Were you actually sound asleep?

Pugsy,

I am showing segmented sleep sessions because my sleep is a mess for years. This is my normal. The central apneas are shown on my machine after I wake up since I had turbinate surgery. I didn't have centrals before my surgery, but that's why I am asking for help with my settings. I always have woken up multiple times a night even when my machine's AHI was usually in the 2-3 range.

If you are seeing central apneas after waking up....they aren't real centrals. They are just false positive flagging because of the irregularity of our awake breathing. Awake breathing is very irregular when compared to asleep breathing.
These machines can't tell if we are asleep or awake....all they measure is air flow and that irregular air flow can and will get flagged by mistake.

If you aren't asleep...they don't count or matter and there's nothing you can do with the machine itself to reduce the numbers.
You have to fix the bad sleep first.

Even if you were asleep when these centrals were flagged...you really can't do much about them with little tweaks from the machine settings other than maybe turning off or reducing exhale relief or in your situation reducing PS or not using PS...and that would only help if the PS was causing the centrals which I doubt it is since you have been doing fine for years.

I think your centrals are a symptom of your poor sleep. If you are awake when they are flagged they mean absolutely nothing other than you were awake and there is nothing you can do in terms of settings to fix them.

Watch those videos...if the bulk of those centrals are awake/arousal breathing related you just have to ignore them.
Perhaps the surgery altered your awake breathing flow rate a little and it's causing false positives to be flagged.

Hi Pugsy,

Last night, I lowered my max IPAP to 15.4 and I slept 7:32 hours last night with one bathroom break and only 3.4 events per hour!

I will leave the settings as they are and see what results tonight brings.
