CPAP Newbie, Intro and some questions

Good Evening,

My name is Alex and I am a 38 year old Air Force vet and Lockheed Martin engineer, and recently diagnosed with sleep apnea (57.1 AHI :( ). I am surely glad I found this board and look forward to getting my therapy dialed in and hopefully beat this thing.

A little history...I had noticed for the last few years a creeping weight gain and blood pressure increase that I couldn't reverse despite eating healthy and weightlifting religiously. My wife complained of my snoring occasionally, but nothing too crazy. I always felt like I got a good night's rest and would usually sleep through the night and outside of occasional afternoon tiredness, never had the debilitating fatigue that I see so many people w/ apnea suffering from. Heck, I would usually weightlift at 7-8pm and would usually crush it.

That was up until 2 months ago, when I started waking up almost every night about 2-3 hours after falling asleep. I would wake up with a fast heart beat almost always right in the middle of a dream, usually with a splitting headache. It would take me hours to get back to sleep. After weeks of that I got my sleep study done and was diagnosed. My guess is I've had this apnea for awhile which has contributed to my aforementioned issues, and it has gotten worse to the point where I now and noticing it.

I've now been on my CPAP for a week or so, and my daily report from the ResMed machine and MyAir app are showing ~1 AHI per hour, which is great. However, I am still waking up most nights within 2-3 hours of falling asleep with the same symptoms as before. After some research it seems my apnea is still showing itself during peak REM. I have an Auto CPAP currently set between 8 and 15 cm. Is it possible that the machine's algorithm is not fast enough to up the pressure when I hit that point in my sleep cycle? I am surprised that my AHI is reading so low despite the fact that I am still getting woken up.

Any ideas or suggestions? I will provide an OSCAR printout in a few days when my SD reader comes in. Thank you so much.

Your story is like many of us. You have come to the right place. Go to the home page and read to topics for beginners at the top of the Home page.

As soon as you can, post a typical night chart with Oscar.

Get used to wearing the mask. Try wearing it while watching TV or listening to music. By all means... keep going. Don't give up on this! It will add years to your life and change your life for the better.

Welcome to the zoo Alex. I'm truly surprised you don't have more replies here but hope they will come when you post your Oscar chart.

Do you feel like your mask is waking you up? Sleeping with a plastic mask on your face does take some time.

Thank you for your service - I am also an AF Vet. in fact retired and happy about it as today it is such a diff world.

Really need to see what is happening via the OSCAR detailed data reports to be able to offer much.
There are some details or data points that the machine gathers that aren't part of the AHI which might point to the airway still trying to close off a bit and you can't monitor those data points without OSCAR.

Thanks for the replies so far, and yes as soon as I get my SD card reader in I will post OSCAR data points to see what is going on.

Last night the same thing happened...woke up 90 minutes after falling asleep right in the middle of a dream, and this was the first time I was gasping pretty good and my heart was racing (fun times). I swear when I woke up I heard some sort of beep or alarm going off on my ResMed AirSense 10 Auto...is that a leak alarm of some sort? I am starting to wonder if I am opening my mouth during REM and losing airway pressure causing it to collapse.

I ordered some Somnifix tape to see if that will make a difference. Ill keep my fingers crossed.

I think you're going to crush apnea and poor sleep! You have a problem-solving, experimental outlook, which puts you way ahead of the game. Your Oscar daily charts may suggest some fine-tuning, but the other factor to keep in mind is that you are still very early in the treatment game. For the lucky few, refreshing sleep comes almost immediately, but for the rest of us, improvement can be gradual. So patience will be another virtue.

About the alarm: there may be "reminders" about replacing filters, hoses, etc., that are turned on. I believe you can check by entering the clinician's menu and going to Options. To get into the clinician's menu, press and hold the round knob and the home button simultaneously until you see the clinical menu option appear.
