Has anyone here had mid-palatal tori removal?

Post by 4638675696 » Thu Dec 24, 2020 1:01 pm

I will cross-post to this to a few related groups. I am 35 years old and preparing for EASE for OSA with Dr. Kasey Li.. I need to have a small torus removed from my palate in order to be an EASE candidate. Dr. Stanley Liu has also confirmed that this would be necessary if I wanted DOME. I'm scheduled for the tori removal surgery this Tuesday and am curious to hear from anyone who has done it and what their experience was like.

Most dentists only remove torii when they interfere with breathing or necessary intubation,
because they GROW BACK.
Removal is not permanent-
-unless dental science has vastly improved since I asked about it.

What is the success rate--and also their definition of "success"?

I barely notice mine and wouldn't be having it removed were it not a requirement for me to proceed with EASE. I assume it isn't a big deal if it grows back post-EASE and the completion of the distraction osteogenesis. I hear that bruxism can cause tori and I definitely grind my teeth. In a perfect world where my SDB is improved from EASE and I am no longer jaw-thrusting during sleep to open my airway, then ideally no tori re-growth.

Here is a copy/paste of some publications about efficacy which should apply to EASE.

Evidence-based literature publications on maxillary expansion to treat OSA
1.) Endoscopically-assisted surgical expansion (EASE) for the treatment of obstructive sleep apnea
https://pubmed.ncbi.nlm.nih.gov/30393018/

2.) Treatment of obstructive sleep apnea syndrome by rapid maxillary expansion
https://pubmed.ncbi.nlm.nih.gov/9871945/

3.) Maxillomandibular expansion for the treatment of sleep-disordered breathing: preliminary result
https://pubmed.ncbi.nlm.nih.gov/15126751/

4.) Distraction Osteogenesis Maxillary Expansion (DOME) for Adult Obstructive Sleep Apnea Patients with High Arched Palate
https://pubmed.ncbi.nlm.nih.gov/28675100/

5.) Effects of surgically assisted rapid maxillary expansion on obstructive sleep apnea and daytime sleepiness

No guarantee that it will work for me and I know people who've had EASE and another with EASE + MMA and still not gained the improvement they're seeking, but many of these folks have UARS and not plain OSA and UARS seems to be tougher to treat.
