Auto BiPap AHI 6.8 first night with Dreamstation!

Hello Forum,

Thank you to those that created this site and to those that contribute. It is great that this exist. I am a LONG TIME Sleep Apnea machine user +-15 years. I had been using a Philips Respironics System One BiPap 760P up until yesterday when I received my DreamStation Auto BiPap. As I had basically no information previously on how I was doing, to now have access to my actual events is quite extraordinary. So I programmed the settings myself as I wanted to start using the machine right away. I used the default settings for AutoB at first (Max IPAP: 25, EPAP: 4, PS Min: 2, PS Max 8, Bi-Flex: On 2, SmartStart: Off, Ramp: 30 Minutes, Ramp Start Pressure: 4) but after 4 hours of sleep decided to increase the EPAP min pressure from 4 to 6 as I felt I needed more. I slept another 4 hours and woke up to these results this morning. 8:43 hours usage, AHI 6.8, Mask Fit 100%, Clear Airway Apneas 1, Obstructive Apneas 45, Total Hypopneas 13, No Large Leaks, IPAP 90% Pressure 16.5, EPAP 90% Pressure 14.0. I know I have Severe Sleep Apnea and I had a little trouble sleeping last night due to Neck Pain. I plan on going to the doctor this week to register my machine and have them tweak it, but any advice on my current settings? Thank you for your thoughts!

Your minimum EPAP is probably still too low.
What were your settings on your old machine?

There is available software so you can see details and do the tweaking yourself.

Thank you Pugsy! If I increase my EPAP further, do I need to adjust the PS min and max? I am just unsure as I am just learning about this stuff even though I have been a user for years. I just thought my doctor knew best about my settings but it is awesome that I can see these now as well. Regarding settings on my previous ResMed System One, I do not know what they are as they programmed them onto my card to override the system.

Without seeing your software reports I don't really have any idea where your settings need to be but I prefer changing only one thing at a time when I make changes. That way you know which change caused what result.

Most likely something around 10 minimum EPAP and I would leave the rest of the settings alone for now.

Or if you still have the old machine...plug it in to the OSCAR software and you will see what the old settings were and how they were doing. Both models are compatible with OSCAR.

My AHI was at 1.9 last night after increasing EPAP Min to 8. I will increase EPAP Min to 10 tonight and see if I do even better! 90% EPAP was 14.5 last night.

I had the best night yet with the DreamStation. AHI at .9 for 9 hours of sleep. Night before i only had 6.5 hours of sleep at was at a 3.8 ahi. For those of you on BiPap, do your numbers vary a lot each day? I know with my old system one, I was averaging about 1.8 ahi each day.

I downloaded the program and imported my SD Card. Unfortunately, I still don't really know what I am looking at. It appears the SD Card they gave me for my dreamstation was used by someone else on another machine previously so I have older data that doesn't belong to me.

There is a way to purge old data from OSCAR and the SD card itself.
Was your machine also used by someone else? Do the total machine hours match your own therapy hours? Do you know how to check machine total hours vs therapy hours?

From OSCAR you can purge the dates that don't belong to you.
It's under "Data" then the Advanced choice.

To remove from the SD card you go into the serial number folder and manually remove old date files. Lots of work if there are lots of files but it can be done.
If the machine itself is still retaining old user files then the next time you put the SD card in the machine it will just put those files right back on the SD card even if you manually remove them.

You might want to check how many hours your machine was used previously. If it was used a lot, but the machine was provided to you as new, you might want to ask your DME to swap this one for a genuinely new one.

The machine itself was only used 68.5 hours previously, but DME gave me a used SD Card that had someones data on it. I will try to remove those old files manually.
