ResMed AirSense 10 AutoSet For Her?
So I encouraged my 75 yr old mother to get a (home) sleep test done. AHI > 38 and Sp02 de-sats to 73%. YIKES.
She lives kinda remote during the summer, far from her “home doc” so I’ve insisted on getting her a unit now. I have the Rx. Only question is this: should I order the “For Her” AutoSet? It’s pretty, lol. Same price it seems. $598 on cpap.com.

Absolutely, get the for Her model!
The AirSense 10 AutoSet for Her is one of the most highly recommended and favourite machines around here. It is functionally identical to the traditional AirSense 10 AutoSet, but has one extra, optional mode.

I have that machine except it is not the "for her" version (and I am a woman). I am looking into selling. I cn go real down in price because I need to sell it. It has only 550 hours and it has the case and a sealed hose. It functions great. The main difference is the fact that the for Her model AutoSet has 3 modes of operation available
Cpap mode
the regular auto adjusting/apap mode
the special auto adjusting/apap mode with a totally different algorithm designed with women in mind
The regular AutoSet (there is no for Him model) has 2 modes of operation available
Cpap mode
the regular auto adjusting/apap mode
So there is most definitely a difference despite what your misinformed DME told you. Now does it matter...not if you don't need or use the special for her algorithm...and I have used it and it most definitely is different. I have used both modes and they are different.
When someone is starting out on cpap and we don't know what pressures they might need or what algorithm might be the best for a person to use we always tell people to get the most comprehensive machine they can because you never know if that special algorithm might end up being a deal breaker for someone or not (and it's not limited to women either...there's no reason in the world that a guy might not end up benefiting from it).
It's always better to have something and not need it than need it and not have it.
The for Her model is like 3 machines in 1....the regular AutoSet is like 2 machines in 1.

Any recommendations for which algo to start her off on? Also, for the mask, I was going to start her with pillows and see how it goes. That worked for me to get used to therapy until I found a better match with the F20 full face (due to my mouth breathing and resulting horrible mouth dryness).

You will be self titrating anyway.
There are a few situations where the for Her mode doesn't work out well for some people above certain pressures because sometimes not enough criteria are present to drive the pressures but we have zero way to know about any of that until we have actual data.
Heck, give the for Her mode a try...if it doesn't work out so great it will be really obvious really fast.
The changes are a little more gradual and someone being very new to all this stuff might fight that fact a little easier to sleep with.
Gotta start somewhere.

Minimum 7 and max 20 (it won't go anywhere upwards without a good reason)
EPR on full time and set to 3
Humidity....middle of the road to start with and see how she does
