Can my therapy be optimized?

Post by thesleepermustawake » Tue Jun 30, 2020 6:32 am

Hey friends

I bought a resmed airsense and left it on default settings. my prescribed pressure is 10.
I use a humidifier.
Does anything stand out to you in the charts?
I still have daytime sleepiness, although it seems mild, like I wouldn't fall asleep driving a car or anything but I just feel that I don't have the energy to get through the day, and sleep doesn't feel refreshing.
I'm very sedentary as a result of this. It seems to come in cycles of severity, like beginning of the year I had enough energy to get through the couch to 5k program, but for the last couple of months, I've really struggled, I yawn a lot during the day, just feel like I could stay in bed all day, take naps. I don't feel depressed though, life is pretty good apart from the sleep apnea. I don't take any meds, I used to take modafinil but it kind of only works for a few weeks, then I seem to feel even more tired than without it. I use a nose spray for my allergies but I do sometimes have a blocked nose when I go to bed even with that. usually one nostril is blocked, but it's not every night. it impacts my life in that mentally, I want to do more but I usually have to avoid very mentally taxing activities because they make me want to take a nap

Try bumping your min. pressure setting to 6 or 7 and see if it helps to even things out (and let us know) after a few nights.

Are you sleeping thru those leaks? Are they waking you up?

What stands out is that your minimum pressure is too low and you have a leak problem.

yeah the leaks are pretty bad - when I sleep on my side is the worst. It seems like a design flaw in this particular mask. the strap on the back of the head keeps riding up as well. and the air flow isn't great unless I sleep on my back
that's the one they provide at the sleep clinic. but I'm open to buying my own. I bought my own CPAP machine because they wanted to give me a fixed pressure one.

any recommendations?

You have the wrong machine chosen in your equipment profile page.
Look down in the R section for your ResMed AirSense 10 AutoSet....that's where it is.

Leaks....if they aren't disturbing your sleep then I don't know that I would worry about them. Not a pretty leak line but large leaks are brief and not prolonged.

I do think you would likely benefit from a higher minimum pressure. Might also help with the leaks if the machine was fitted to a higher minimum to start with. When you fit it at 4 cm and it goes to 14....it's not going to seal well.
I am thinking something around 7 or 8 minimum might work better and maybe with a more optimal minimum it won't need to go to 14 as much.
