Put me on the list of managing my hose with pillows. I would freak out if I woke up and something was hanging over my head. Lol.

Metaphorically?
Physically.

I'm somewhat claustrophobic. The sleep study and tritiation nights were pure torture being hooked up to all that stuff.

I'm like the person who "Snuggles" up to the hose... Get a hose cover, something fleecy and soft and just make sure the end that goes to your machine is secured so you can't yank the machine off your night stand into the floor if something crazy happens like you have a bad dream or fall out of bed.
I have my XPAP device on the night stand with just enough hose to get it looped over my headboard with a few inches of wiggle room so there's no tension on the hose where it hooks up to my machine. I Velcro it from that spot to my headboard. The rest of the fleecy covered hose is for me to roll and toss and turn with all night. I've been on PAP now for going on 15 years I think? Not 100% sure, maybe more. I hated the hose when it wasn't covered buy something soft, but with it covered, it's just another part of my bed linens and not something that concerns me. I get situated and flip the extra hose behind my head or to the side and that's it.
I may be a lot less of a tosser and turner, but that's just me.
This is one of those questions that is going to be 100% "your mileage may very" it's very personal and what works for some won't work for others and vise versa...
Best of luck.

After some further thought, I think I will string my hose back up on the hook thingy above my head. I don't think it makes a difference to me not having it lifted up, in terms of comfort or positioning, but I'm worried I might damage the hose by sitting on it/laying on it/getting it pinched in the mattress, etc., if it's not up out of the way. I don't want to crunch it or poke a hole in it--seems a little fragile to me.
So, full circle.

I might damage the hose
Fundamental CPAP rule: Always keep a spare.
BTW, the hoses are tougher than they look.

I have lain on mine a few times and did not do any damage to it. My hose runs up over the head of the bed and has done so since I first started four and a half years ago. I have only replaced one hose, and that was due to a small leak right next to the rubber end of the hose that connects to the mask.
Growing old is mandatory, but growing up is optional.
