APAP adjustments

Post by desertscorpion » Sat Aug 31, 2019 7:30 am

Been using ResMed S9 Adapt for a couple months and seem to be stuck in the 2-3 AHI range and based on a typical night result (attached), was wondering what adjustments I should try to see if I can get AHI lower? (Is Leak Rate OK? Should I lower max PS?)

Leak rate is NOT okay if this is what you see often.
This much large leak can definitely impact sleep quality. Are you sleeping soundly or do you wake up often?

If you lower PS max then if you are using this machine to treat central apnea then the machine might not be able to adequately deal with the centrals.

If you were asleep and the flagged events are mostly obstructive related the minimum EPAP is what I would look at changing first.
Looks like you are pretty much running at the machine default settings from the factory....minimum EPAP of 3 isn't able to quite deal with the obstructive stuff.

Minimum EPAP would be my first choice to tweak if it were my report and I was asleep when those events got flagged.

When you fit the mask...are you fitting it at the 3 cm EPAP and 6 cm IPAP which is what you start out the night with at these settings?
If so...when you go higher the fit isn't going to work. Needs to be fitted when the mask is at the higher settings.
What works well at 3/6 is going to blow out when the pressure goes higher.

I would say increase minimum epap to 6 and sort out your mouth breathing issues
