AHI is 15-20 each night mainly Clear Airway

Hi,

Last night was similar to most other nights. I have AHI around 20 and Clear Airway 18.07.

Here are numbers
AHI 20.09
Large Leak 2.25%
Clear Airway 18.07
Obstructive 0.46
Unclassified Apnea 1.47
Hypopnea 1.47
RERA 0.18

Each night the Clear Air way apnea accounts for almost all the AHI. It happens throughout the night but mainly during the 2nd half of the night. I would say about 80% of clearway apnea occurs in 2nd half of the night. In fact the longer I sleep the more I get. If I sleep only 6-7 hours my numbers are not that bad, AHI around 5-7. If I sleep 11 hours or so like last night I get the high AHI due to clear airway apnea in the second half of the night.

Does anyone have any ideas ? My clear airway apnea did drop someone after switching to a full mask. My pressure is 12-16 and averages 14 the whole night, usually won't get to the 16.

I'm thinking of having my doctor do a full sleep study to come up with the reason.

Any help much appreciated.

You need to go back to your doctor for another machine as the machine you have will not treat your CA’s we could possibly reduce the CA’s with fixed pressure and turning off EPR but first stage is talk to your dr your CA’s are too high I assume you have the Resmed Autoset and not the CPAP as described in your signature otherwise you would not be able to provide any data

Thank you for the response. I have updated my machine as it is the Resmed Airsense 10 with auto set.

Can my Dr see these clear airway apneas ? I don't see them on the iPhone App that comes with the machine ( MyAir ). I do see them on the sleepyhead software though. I have appointment with Dr. pretty soon and I will bring it up.

Thank you for the response. I have updated my machine as it is the Resmed Airsense 10 with auto set.

How were you diagnosed? What was the diagnosis? How were your machine type and settings determined?

For future reference, turn off the Mask Pressure, Snore and Flow Limitation charts. As you get down to fine tuning your CPAP therapy, someone may ask you to turn one or more of those back on. But for right now, they only make the more important charts harder to read.

Go back to your Dr you need another machine, your EPR is 1 your pressure is fairly tight we could try fixed pressure but I am not sure it will help, how long have you been using an APAP machine ?

First thing I would say is to turn off EPR, it may not help the centrals, but it's worth a shot.

Second, get rid of the mask pressure trace, (grab it and pull it down) then you can make the other ones larger. You don't need pressure AND mask pressure, they're redundant.

I was diagnosed using at home sleep study, I only had around 10 AHI in the at home study and was diagnosed as mild sleep apnea.

I switched to new doctor due to relocating from PA to TX, he looked at my machine data and saw that I still had AHI even higher than I was diagnosed before despite being on a machine. He asked if I had gained any weight, I said maybe about 10 lbs. He thought maybe the machine wasn't working properly so gave me prescription for a new machine and said to try it out for couple of months and get back to him. So that is what I have been doing. Originally he set me at 8 to 16 pressure, I upped to 12 to 16 thinking that might fix my problem but I pretty much got same results. I will turn off the EPR tonight and see what happens. I have appointment pretty soon with him so hopefully he can get to bottom of issue. I hope that he can give me in lab study to see if that finds any issues.

I have been using about 4 years now. I don't have headaches in morning and my blood pressure is pretty good. Am overweight but working on that.

Given the high number of clear airway apneas, I would also change the minimum back to 8.0.

It might be interesting to see how long the clear airway apneas are. If you click on the Events tab, you can see a list of each one.

I asked how long you had been receiving treatment to see if you were a new user, as you are not then this is not treatment emergent CA’s. Have you just got this machine ? Do you live at altitude ? What were you using before ?

Looking at the numbers the clear airway averages around 15 and the high was 24.

Last night I turned of EPR and it cut my clear airway in half, so this looks good.
