from cpap to apap worse for me

Post by jpaul321 » Tue Sep 26, 2017 2:29 pm

I went from resmed s9, which worked well for me to a dreamstation apap. My resmed was set at 6.5 and my dreamstation is set at between 6.0 and 8. My AHI scores after 5 nights with the dreamstation are all well above 10. I've been told it will take a month to get used to the new machine. I'm thinking about going back to what works. Any advice is welcome.

I found that APAP did not work as well for me as CPAP. But I never was willing to suffer through a month of poor sleep to see if I can a better result. CPAP I am running an AHI of 0.4-2, and an RDI of about 5. It's good enough for me to feel good.

Then set the DreamStation to cpap mode at set it at 6.5 cm.
Just because you have apap mode available doesn't mean you have to use it in apap mode.

Or try to figure out why the AHI is different...starting with what the AHI is composed of..what is the breakdown into each event category?

How is your sleep quality? Waking up often and/or spending much time awake with mask and machine on?

Logic = Go back to what works....Who set the Dreamstation, and why?

Thanks for the replies, I get mostly hypopneas, the settings for the dreamstation were prescribed by the sleep Dr. I downloaded the sleepyhead ap and since I never bothered with these things there is quite a bit I don't understand.

The definition criteria is just a tiny bit different for hyponeas between the 2 brands but without seeing some actual reports to compare to I don't know if that tiny bit of difference would account for the increase in AHI.

Plus even though the 6 minimum pressure isn't all that much difference from your fixed cpap pressure of 6.5 it could allow some apnea events to happen if you spent much time down near that 6 cm. Sometimes 0.5 cm can make a rather substantial difference in results.

I always had more flagged events with my Respironics machines than I did with my ResMed machines. Part of it was likely the definition criteria and part may just have been the difference in how each machine goes about doing their job.

What you might do is just set that minimum from 6.0 to the 6.5 you were using and see what happens.
You have several options though depending on what you want to accomplish.

Data, using the links below, is best to understand and make informed changes.
Your dreamstation machine is capable of emulating your resmed machine, and has more functionality. So you need to understand how the therapy differences are effecting your results (data and satisfaction). A small amount of effort is all that's needed.

Don't have any machine advice that hasn't already been mentioned. Just noticed my account name is listed as "also posted as" under your name and wondering what our computers have in common that made it connect us. Sorry, just still trying to figure out the exact reason the program used for this forum does that. Hope you get your treatment working well soon.

It's just based on IP, whenever you get on the net, you could go to http://whatismyipaddress.com/ and see what your IP is for that session, you'll likely find it changing on a regular basis.

When you sign in, the clunky forum software remembers what IP you signed in from, 10 years ago, when this software was released, there wasn't the shortage and re-use of IPs that there is today. Having run out of IPs, we have to share much more than we did before, so other people get temporarily assigned an IP that you used, post as guest here, and poof... "also posted as"

I was wondering about those "also posted as" things as well. Thanks for the explanation!

Two years or so ago, or when this first started happening, I was accused of being several different people. I didn't know what was happening at the time, and gave up explaining that I didn't know who those people were... well, except one person... I liked her. It was frustrating and still is. This should prob be made into a sticky so that new people who seemingly get assigned several different names, know why.
