Does anyone know how the Resmed myAir connection works?

The manual says it uses the cellular system. How does that work? Does it dial in? Who. pays for the use of the cellular system?

The DME pays ResMed for service for the machine, and then ResMed pays one of the cellular carriers for service. I would imagine it uses a tiny amount of cellular data, or possibly text messages. You don't need to add it to your cellular plan
