AHI at 7ish... what adjustments should I make?

Post by amyela » Sat Oct 01, 2016 3:41 pm

After many years of using a mandibular advancement device, I'm trying to get back into the swing of using a CPAP machine. The first several nights I used it, my AHI was around 3ish, but I now think those numbers were artifically low since I was spending a lot of time awake as I was getting used to it. Now that I'm actually sleeping, my numbers have been around 7 or so and I'm wondering what the best adjustment to make would be. I suspect I should up my pressure to lower the number of obstructive apneas and hypopneas, but wanted to get some opinions, especially since I'm not sure a pressure adjustment would affect the number of centrals, which appear to make up most of my events.

I'm particularly interested in thoughts about what may be causing the central apneas? Although I don't have access to the results of my diagnosis and titration sleep studies, I don't remember my sleep dr. ever mentioning that I was having centrals when observed in the lab. I tend to wake several times during the night due to sensory processing issues, which is a separate issue from my sleep apnea, although there are probably ways they interact. Perhaps the constant waking and falling back asleep is causing them?

Let me know what other information might be useful. The screenshot I'm posting here is a fairly typical representation of my recent Sleepyhead reports. I know that lowering the AHI will probably be a series of trials and errors, but I just want help to make an educated guess about what to try first, if that makes sense. Thanks in advance for your thoughts!

(Also I'm not sure if I'm doing the imgur upload correctly... ?)

Pressure looks low. I would try min 10, max 20 and use the c-flex (I speak resmed I'm afraid) if you need it. If you're using a ramp I'd turn it off. See how that goes.

do you not even bother to look at the chart? it's a cpap, not an apap.

more pressure may exacerbate the centrals, but it is worth trying.
