AHI creep

Howdy all,

I started pap therapy a little less than a year ago. We started out with an auto cpap and self titration because I didn't sleep well at the sleep lab.

That was very helpful but something still wasn't right and I "broke down" and went for my titration night. I was prescribed a bipap with 16/20 pressures. My RT programmed in a pressure support of 2 on my Aircurve. It was wonderful. My AHI dropped to under 1 and stayed there from the first night of bipap in September until sometime in December. I had stopped pulling the card and checking things out, so it wasn't until a very sleepy afternoon in January that I really noticed. Apparently I notice daytime sleepiness if my AHI is greater than 1 on a consistent basis. Last night it was 5.

My RT changed my inhalation pressure such that it ranges up to 25. That was very slightly helpful. It has only approached 25 once; it usually tops out around 23 or so.

I upped my exhalation pressure to 17 and am considering raising it some more.

She also got my PCP to order an overnight oximetry and it was normal.

I'll be getting in touch with the RT again and see if she has any further thoughts. The lab has no sleep doc and my RT is at the DME.

In the meantime, does anyone here have any thoughts?

zoom in, look at what the lower pressure is when you start to have those clusters of events, and raise the lower pressure (min-epap) to just above that.looks like it might be 18, maybe a bit more, a bit less, hard to tell
