Pressure change going from a Nasal cradle to pillow mask

Post by Jdougc74 » Mon Nov 04, 2024 6:55 pm

I need some insight please. I am changing to the ResMed P10 nasal pillow cushion from the ResMed N30 nasal cradle cushion.
I have received some god tips about using lanoline to help my nose not be irritated so much.
But I forgot to ask if I should lower my pressure a little bit since the air might be better directed into my nose. I use constant pressure of 10.2 on a ResMed AirSense 11 in the cpap mode. Should I try 10 or 9.8, not making a huge change obviously?
I use a 5" long piece of Kinesiology tape, 4 inches wide, and a chin strap also. I just got the Caldera soft neck cushion, but it is a bit small. Even with using the last little bit of velcro, there is too much pressure on my neck. I woke up mumbling with a sore adam's apple. Hopefully some velcro extension will help.
Thanks for any help in the pressure settings. My breathing was better even though the neck collar was too tight. I took it off after waking up and had a bit more leaks.

I wouldn't go reducing the pressure just because of the mask change any more than I would suggest increasing the pressure when going to a full face mask without evaluating all the parameters involved.

I was wondering if the pillow type cushions direct the air so much more accurately, that a bit lower air pressure would provide an equivalent apnea relief as other types of cushions at a higher pressure. In my particular case I seem to have a better night of rest when the pressure is lower. Originally in 2018, I was on a fixed pressure of 15 with a full face mask. I did not have Oscar until this summer, but I had loud leaks back then. With my newer nasal cradle cushion, I am able to lower the fixed pressure to around 10.
Last night was worse without the collar. I can't use the tight soft neck collar until I get a velcro extension.

You do understand that OSA events relate to a collapse in the airway but not the airway in the nasal cavity itself???

Pugsy,
Yes I do realize that OSA events relate to the collapse of the airway, but not the airway in the nasal cavity itself.

I switched to the P10 pillow cushion last night. My nostrils flared out considerably, wow!
I used a soft cervical collar as the vecro extension came in. I used my mouth tape, no shin strap, very comfortable.
My flow limitations seem ok. Not sure if I need to turn on EPR. This is only one night, so I will see how it goes.

2nd night of using the P10 pillow cushion. My nostrils still flared out considerably! Much more than the Nasal cradle cushion, the N30.
I used a soft cervical collar, mouth tape, fairly comfortable.
A fair night, only woke up 2 or 3 times, not sure why. I shut the cpap machine off only once, to um. well never mind.

Another night of using the P10 pillow cushion.
I used a soft cervical collar, mouth tape.
I am trying to lower the Flow Limitations, even though the AirSense 11 isn't recognizing them. I am slowly raising the pressure by .2 or .4 cm ever other night with some success.
I tried using EPR but that seemed to cause more arousals. I might try it again though as I get used to the new pressures.
I attached last night with some zoomed in pics to highlight possible some Flow limitations.
Thanks for any insight.
