I've been thinking that it's the equipment
I'm in England so it's all done vis the NHS, I'll have to wait for my review (which I beleive is soon) and see what they have to say I guess

I'm sure it's too tight, I just cant seem to get it right anymore
I've not replaced or changed anything since I've had it (a year)
How often would/ should you change things??
