I use a 5 minute ramp, at therapeutic levels, just to give myself a few minutes to rearrange myself and the mask to get comfortable and ready to sleep. I tend to hold my breath while tossing and turning (which I do a lot of during that 5 minutes), and I don’t want those to show up as events

I don’t see this as an alternative to CPAP anyway - reducing events by 60% is great, but that leaves the other 40% that need to be treated. The main difference between the two machines is the pressure support/EPR - you have PS of 4 on the Aircurve compared to the equivalent of 3 on the Airsense. The only thing I would change is the ramp - 45 minutes is way too long in my opinion since you really get no therapy at such a low pressure. Personally, I use a 5 minute ramp (just to get comfortable and to be sure everything is in place), and even then I use a ramp pressure right around my minimum ...

Depending on your Aircurve settings, it might be possible to set up the Airsense to provide a reasonable approximation of the same therapy. My original sleep testing and machine purchase was under private insurance, but the switch to bilevel was under Medicare.

I'm just wondering what they are going to want for documentation. I completed the questionnaire, but honestly don’t see much value in it. The questions were simply too general and didn’t point to reasons why people might find CPAP therapy to be easy/difficult or why masks might be uncomfortable.

The flow limitations could be causing micro-arousals too, which might lead to not feeling refreshed but still having low treated AHI.
