My personal opinion is to go with a minimum of 5 and no ramp until you are comfortable with the machine. The top end probably won't really matter that much. I've washed all the parts several times with no problems. I put them in the top rack and we don't use the "sanitize" option.
And yes, the collar slips and has to be slid up the bottom to fit.

I found a difference going from small to medium with the nuance and medium to large with the airfit. The holes are bigger and it feels "softer" and less jet like.
Having said that, I just switched back to the medium on the airfit and am finding the fit a bit more comfortable than the large.

If he's not using the large pillows he could try the next size up. I went from small to medium with the nuance pro, and then to large with the P10. The bigger holes made a difference in comfort for me.
