You might try looking up by city business license or by the state regulatory agency for DME's (E.g. California Department of Public Health). Medicare and Medicaid may also have a database of authorized vendors that can help you identify the owner.

I agree with Zonker that you need medical attention. Left arm pain and tingling, in men especially, can be a serious sign of a heart attack. You should call 9-1-1 as you say the pain is severe.
