You can turn the knob on the forehead rest to relieve pressure on your nose. I find I can run the mask pretty lose up top without leaks, but the bottom has to be tight in order to avoid leaks.

Just wanted to post a followup in case it helps someone.

Turns out I needed a lower exhale pressure. I titrated at 20/15 at my follow up study and will be switching to a BiPAP.

Last night I got an AHI of 2 with my CPAP by running a pressure of 20 with C-FLEX set to 3.

While I am waiting for the BiPAP (to run 20/15) the sleep tech set my CPAP to 20 with C-FLEX = 3. I hadn't been using C-FLEX or A-FLEX at all. Thanks guys! I didn't realize that the BiPAP version was so similar to what I already have (and that the same software would work, etc). Definitely the way to go.

I currently have an M Series Auto with Encore Pro and Encore Pro Analyzer that I like a lot. What BiPAP machines have this level of software/reporting?

Have you self-titrated for tummy and side sleeping? I was titrated at 19 cm on my back. I used my Encore software to titrate myself on my sides and tummy. 8.5 cm works fine and is much more comfortable. I just have to avoid rolling onto my back. Mirage Quattro works great for me -- leak free even at 20. My hair is shaved short so I do put a paper towel under the lower neoprene strap in the back to avoid skin irritation, but other that that it has worked great. I don't remember all the details of why you are on the auto and why you are trying cpap pressure of 15 but if that report is typical for you on 15cm then it obviously isn't doing the trick. If it were me I would switch it back over to auto and let it max out. Keep the minimum at 15 or so. You say y...

I think apnea in the absence of significant desats can still be a problem. Stress to the heart may still be present and also the arrousals may prevent you from getting a good night sleep. Hadn't thought about stress on the heart, but you may right. I would have thought that it would only work harde...

You are having a lot of events but not having a huge drop in O2. Not everyone will have huge desats with events.
