File a complaint with your state insurance board. In Michigan you can do it online at http://www.michigan.gov/difs/. You wouldn't believe how well this works. I tried the pillow masks, but the force of the air in my nose was way too much. The nasal mask I've tried is a Fisher & Paykel FlexiFit. It is much more comfortable than the FFM, but I don't keep my mouth closed.

One thing I forgot to mention...
I have the mask on pretty tight. It's leaving lines in my face and forehead. But I can live with face lines for a good night's sleep.
