I just confirmed with Phil at mycap.org that you do need the card reader driver installed as well as the smart card reader service running for the application to work.

No good. The problem isn't downloading the application but rather in trying to run it once installed. Install the program, click on the "Shortcut to MyCPAP Card Utility" icon on the desktop and the only thing that happens is a small window flashes for about a 1/4 of a second or less and then nothing.
