Re: BarryKrakowMD New Sleep Newsletter
Here's a link to the Discussion thread: https://fastasleep.substack.com/p/premiere-discussion-event/comments A few other responses: We love Savannah. Thanks. ASV in my experience is outlandishly more effective than all the other devices I've tried, which is a common refrain among our patients who al...

Re: ASV vs CPAP for Chronic Insomnia
Some comments: 1. Regarding price, a lot of used or even new ASV devices are sold at markedly discounted prices on direct to consumer websites. 2. Qualifying for ASV is the way to get insurance to cover the device, but remember insurance coverage doesn't tell us how much the insurance covers. Qualif...

Dr. Krakow Updates
Just completed a TEDx talk in Albuquerque on Sept 9th on my favorite topic of "why do you wake up at night?" Delves into "complex insomnia," the relationship between sleep breathing events and chronic insomnia. Also, probably by the end of the year, we should be finishing up our randomized controlle...

Re: Dr. Krakow, salesman extraordinaire?
As briefly as I can: 1. No clue where this guy is coming from on the credit cards. We see all patients and practically all insurance. The one time credit card up front comes into play is when someone has a huge deductible and we ask for coverage in that situation beforehand. Or, we also ask for VIP ...

Deep Sleep
I'm of the opinion that normal, healthy sleep achieved through optimal non-drug sleep treatments is going to result in a general increase in delta and REM sleep. I'm not just referring to rebound; I mean sustained increases from pre-treatment levels. However, there are so many factors that influence...

Cause vs Epiphenomenon
SWS, I'm not fully sure I understand the mechanism you are proposing, so I'll start by asking for more info, but I'll finish with a couple of thoughts they may be relevant to your point. Do I believe that flow limitatoin could be the sign of something else, and that if you could treat the "something...

Anxiety, Flow Limitations
FL vs UARS In some of my earlier posts, I raised the question about what is normal sleep, and I offered the hypothesis that a normal sleeper might have little or no daytime sleepiness. Recently, we started looking at patients who claim they are normal sleepers, and in the first 3 individuals we tes...

Reductionism, ENT, and OAT
The dictionary.com definition of reductionism: 1. the theory that every complex phenomenon, esp. in biology or psychology, can be explained by analyzing the simplest, most basic physical mechanisms that are in operation during the phenomenon. (italics added) Stating that something is “reductionistic...

Change of Direction in Golf or Breathing
I appreciate all the support and open-mindedness to our ideas, and I'm very appreciative of SAG's provocative attempts to raise our consciousness or insights on respiratory physiology. I just wish I could understand half of what SAG is trying to communicate, but I'm certain the fault lies in my own ...

Caffeine
I drink one cup of cocoa about 10 to 15 mg (max) of caffeine most afternoons and have 1 piece of chocolate around dinner time (about 5 mg). Both products are dark chocolate, and I'm impressed that I seem to notice a definite boost from both ingestions. My goal would be to need no caffeine whatsoever...

BEACON System and Caffeine
I rarely recommend anyone go off coffee or other caffeine products unless or until they have fully evaluated and treated their sleep disorders. Caffeine is the miracle drug of modern civilization, probably having saved more lives, enhanced more productivity, and treated more mild to moderate cases o...

Loop Gain vs Anxiety
I always assumed that loop gain was part of the equation, because it seemed to me that an anxious patient would hyperventilate off more CO2. In fact, on some diagnostic studies, where it's easier and cheaper to measure CO2, we do see relatively lower CO2 baselines in some patients who subsequently h...
