Pugsy, I would like to return to a point in the discussion.
Whatever the cause, at some point, asleep or awake, I no longer breathe. This is a fact. Which implies that my throat is closed. So CPAP remains necessary to help open this closed throat?

Thank you Pugsy. If I understand you well, most of my OA's/H's happen when I am not fully asleep but mostly awake. It may mean that my problem, a crappy sleep, is out of the scope of CPAP. ? I regularly have these clusters which represent the bulk of my total AHI. Putting on a cervical collar does not change anything.

My "bad" cluster lasted 20 mn with 11 OA's, each one lasting more than 20 seconds, without mentionning several miserable phases not mentioned as OA by Oscar.
When I look at my O2 reporting at this very moment, it looks great ! Never below 95, with an average of 97.

Obstuctives are prevented at EPAP, IPAP has nothing to do with them, because you can't get to IPAP if you don't trigger an inhalation, and not during 'exhale', during the rest period post exhale before the initiation of a new breath. My best guess is you have crappy sleep from some spontaneous arousals (not related to any airway issues) but I have no way to know for sure and there's no way to identify what might have caused the spontaneous arousals. To my eye none of those look like real asleep breathing. Looks more like awake breathing to me. Thank you Pugsy, the fact that my AO's are fakes, would be consistent with a situation where I am, very often, half awake, strugling and fidgeting in bed.
