UARS BIPAP Pressure Settings

Post by steve_zissou » Sat Aug 29, 2020 2:19 pm

Hi everyone,

I was wondering if anyone can give me some guidance on how to proceed with my BI-level pressures for UARS. I had a titration done, but I don't think it was very successfully completed (also, I got very little sleep during it). Here were the highlights:

I have a few questions about this. It seems like halfway in the night I suddenly stopped having RERA's even though my pressure settings remained the same. Should I infer from this that maybe the pressure I was on at that point was sufficient, or should I go higher? I just find it strange that the same pressure setting gives me a lot of RERA's for the first half, then no RERA's for the second? Also, you might see that the 10 / 6 setting seemed to clear my RERA's. However, I was only on this setting for about 7 minutes of sleep, and had no REM sleep, which makes me doubtful. What do you guys think.

Anyways, I've been fiddling around with my settings and looking at OSCAR since then. Here was yesterday on a fixed setting of 11 / 6 (PS 5)

Here is a different night on a more variable pressure (PS 4.4, with a range for min/max)

I think you've interpreted the implications of the study correctly.

In any case, I would do min EPAP of 6 and VAuto mode.

PS is harder to know; maybe just choose a setting within that 4-5 range and keep it stable for a week or so?

It's the PS setting that you're using to resolve the RERAs, and whether that's working on not you'll need to rely more on subjective feel (do you feel rested in the morning, for example).

Thanks slowriter! Here is some data from last few nights...

How do these look? Is the main thing I am concerned about supposed to be the flow limitations?

The numbers in general look good.

For UARS, you do indeed want to reduce the FL as much as possible, and the data you posted shows those settings are doing just that.

Any subjective impressions?
