Machine limits

I have the PR System One 60 Series. The results from my 2nd sleep study want me to be at Bilevel 28/19, it looks like my machine
only goes to 25. I think I might be in trouble. Do you think I should just turn the machine all the way up, or just leave well enough
alone for tonight. It is good that the machine is only a rental so it can be returned. I was wondering what kind of machine I should tell
them I want for the higher pressures.

Any advise would be appreciated.

An older model from Respironics is still sold for this very problem. It is an AutoSV advanced unit with the model number 1042906. It goes up to 30 for a max pressure. It is normally a little over $4000. I can probably find you a new one for about $1000. I will let others chime in if they know of other machines that go up to or over 30. You can read more about it at this link. It came out in 2009 so not up to most of today's standards as far as data and software compatibility.

Mark since this is a older model is it brick that doesn't collect data? I wonder what my insurance company is going to do.

Maybe others will have a solution. You are approaching ventilator levels and that type of machine may be needed. (Machines like the Trilogy100 go up to 50 cm H20) But I am sure the respiratory techs can give you good advise as to what your options are. It does have some form of data card others here may be able to tell you more about what that stores. (Looks like I guessed wrong, list is $5600).

It does look like Phillips Respironics has a unit called the BiPap A30 but I can't even determine if it is marketed in North America. It is a great looking unit. Look at this link. That is about the nicest BiPap I have seen.

It looks like the higher end ResMed units like the S9 VPAP ST-A may handle it. I am sure they will find a good solution for you.

The older Respironics AutoSV advanced unit with the model number 1042906 is a full data machine in regards to the data that was gathered when it was top of the line.
It uses a Smart card and not a SD card and there is a special card reader that is needed (but is easily available) as well as software which is available. Far from being a brick.

I use the Trilogy 100 and unless updates have changed it, mine only goes as high as 36 cm H20. I'll check the one I have now (more updated than mine) and see if any of the modes have a maximum IPAP above 36.

Edited to add: I just looked at the Respironics website link to the Trilogy 100 (thanks Mark) and found they HAVE made some updates. The website claims max IPAP of 40 cm H20 now. It also indicates a nice update to AVAPS that will be great for me! I hope my machine has these updates when I get it back!

I attempted last night to take the machine from 19 to 25 and i thought my ear drums were going to pop out of my head.
I put the machine last night to 23 and 17 I figure I will work my way up. I can't believe my AHI went down to 1.31. That is
the best it ever has been, and NO vibratory snore events. Only problem is I had 2 hours 7 minutes in large leak. My mask is
4 months old maybe I need replacement.

As I understand it, higher pressures make it more difficult to control leaks. 4 months isn't much for a mask, but maybe you need a different style.

I am using the Trilogy at 18/9, with spikes up to 23. I use the F&P 431 FFM. I know that many people here despise this mask and a lot simply cannot use it. For me, it has been a fabulous mask, both comfortable and good at controlling leaks.

With the pressures you're trying to get to, I don't know. Full face masks definitely make leak control more difficult.
