ASV Questions before sleep study

I'm currently using an auto bi-level CPAP machine the controls my obstrucive events pretty well.

Recently I'm having more and longer clear airway events and am considering a new study to see if an ASV type machine will do better.

So, I have some newby type questions.

Should I ask for a complete new study with a follow on ASV titration if that is appropriate

Or

Should I ask for just an overnight ASV titration?

For either scenario, should I consider skipping my CPAP for a night or two to get rid of residual effects and make the study see my underlying condition? If so, one night or two? I don't think I could tolerate more than that.

I can likely get any or all of the underlying data recorded in the study. What should I ask for?
The system being used in the center which I would use is built by ResMed so I'm not sure what is available in tems of breathing data. I'm used to the reports that come out of PR System machines and do find that data very useful.

My EPAP/IPAP numbers are 15.5/19.0. Does anyone have experience going from pressures similar to this to an ASV machine?

I would just ask for an ASV titration. Note that the central apneas are increasing in frequency and duration. It can be difficult enough to adjust to the changing ASV pressure, so I would not push it with a split night study (BiPAP and ASV). Just go for an ASV study. But remember, if your AHI is not over 5 and half of the apneas are not Central, then they might not approve the ASV titration. In that case the logical next step would be to try a BiLevel titration. The thinking would probably be that the central apneas are due to periodic arousals that a BiLevel machine would address.

Probably no need to skip CPAP .. unless you want to be REALLY tired for the sleep study.

I usually just ask for the summary pages from the sleep study. Unless I think something was missed.

You should not have any problem. But wait. Do you have CPAP or BiPAP? I'm confused.

New question as I get closer to getting an ASV machine.

The Respironics DS950HS machine has a short paragraph in their brochure that indicates a way exists to connect a pulse-oximeter to the machine and get the data integrated.

Does anyone know what oximeter they are talking about and how it connects?

There's an expensive Respironics pulse-ox that attaches on the back where the modem/bluetooth/USB/etc. connect.

A quick search shows prices of approximately $330 list just for the oximetry accessory, not including the actual probe. I couldn't find a price for 1075383. This PDF makes it look like the 1075383 is the connector between the accessory and the sensor, so it seems necessary. On the plus side, it looks like the oximetry accessory is also a broadband adapter.

Here's Masimo's page for adhesive and reusable sensors. The clip oximeters hurt my fingers after more than one night, so I'd probably go for the soft reusable one. I found that one for $230 online.

So you're looking at over $600.

The Respironics price for Masimo HPLP oximetry module 1075383 is $364 as of today.
Looks like about $1,000 in all

I agree with what John Fisher said and would only add that be sure to get a PR Bipap autoSV, not a ResMed ASV. PR gives you so much more control over your breathing patterns via the the three Auto-Digital Trak Sensitivity features. Much more than the equivalent ResMed models... I happen to know you have an inquiring mind, so you will most likely appreciate those features. Let me know if you want the PR AutoSV clinician's manual and I will shoot you the link.

I like my ResMed AirFit F10 FFM - reasonably low leaks for my ASV therapy. I'm currently using a PR S1 AutoSV 960P Advanced. I also keep a ResMed S9 Adapt as backup. I use a heated Hibernite hose. Still rockin' with Win 7 by using GWX to stop Win 10.

Thank you for the kind offer. The DME who supplied my 960 left the clinician manual in the box so I have the complete manual.

I agree with JohnB, 960 is a really superior machine. I've not had the chance to compare it to the ResMed equivalent except for two nights in the sleep lab which uses ResMed equipment. The ResMed seems to be slightly more pushy on inhale but as you might know, i'm pretty insensitive to pressure changes (I frequently sleep through 2 or more hours of ventilator mode with my pressure doing 10 to 24 cm H2O breath by breath.)
