S9 AutoSet ?

Post by The Dreamer » Sat Feb 25, 2012 1:47 pm

Hi all

I'm looking at a new S9 AutoSet, I don't understand the stament below. My min. setting is 11.5 H20, does the stament mean that it will not treat any apneas that I may have? since my min. setting is already set above 10 H20.

Apneas
The APAP algorithm only treats apneas up to 10 cm H2O and does not differentiate between different
types of apneas.
The enhanced AutoSet algorithm detects both obstructive and central apneas. If an apnea occurs, the
device responds appropriately.

Thanks

Help us please. Where do you find that statement? Would like to know so we can see the context.

It came from a Res Med S9 AutoSet manual on page 7

That statement is true of the S8 AutoSet and the S7 AutoSet. Both were earlier generations of AutoSet. The enhanced algorithm used in the S9 generation AutoSet models do, indeed, attempt differentiation:

Are you sure that manual is for an S9 generation?

This statement Came right out of A S9 clinical guide on page 7

That statement is made at page 6 at the top of the page.

I think the statement endeavors to to distinguish Resmed's latest algorithm from other APAP algorithms as follows.
The Dreamer wrote: This statement Came right out of A S9 clinical guide on page 7

The APAP algorithm only treats apneas up to 10 cm H2O and does not differentiate between different types of apneas.
[ meaning APAP algorithms in general]

The enhanced AutoSet algorithm detects both obstructive and central apneas. If an apnea occurs, the device responds appropriately.
[meaning the S9 algorithm in particular]
The S9 definitely attempts differentiation of central versus obstructive apneas. The PR System One does as well. Most APAP algorithms do not.

I believe the cenetral-less algorithm belongs to the S9 Escape.

That's true---and it's news to me. A side-by-side comparison of features is available on the specification tab here:

LOL Glad you explained that...

Very interesting. Looks like a significant difference that we've missed in all our prior comments concerning differences between the Escape and Autoset models. I always thought the Escape Auto did the same thing as the Autoset but without the data recording capability. But this shows if you need a pressure above 10 in APAP mode you MUST have the Autoset.

If you're referring to the S9 AutoSet being mandatory over the S9 Escape Auto in that scenario, then I would have to disagree.

The previous-generation S7 and S8 AutoSet algorithm (named "A10") doesn't respond to apneas above 10cm either. And yet there is plenty of clinical data showing that A10 algorithm treats patients fine who need pressures above 10cm. That A10 algorithm responds to precursor events such as snore and flow limitation to adequately prevent obstructive apneas. That's probably the algorithm used by the S9 Escape Auto.

I would agree that the enhanced S9 AutoSet is preferable in that above-10cm scenario you mentioned. I would prefer the enhanced S9 AutoSet algorithm both below and above 10cm. Obstructive apnea patients who don't present many snore or flow-limitation precursor signals might actually be better off avoiding all APAP algorithms...
