Managing Pulmonary Hypertension

Good morning wise group members,

At the end of an extremely long week for my family, my last minute panic of planning my upcoming conference was interrupted by an emergency hospital visit for my wife.

Little back story, my wife has an atrial septum defect repair following the birth of our second child in 2002, they repaired her birth defect and everything has been ok since. Recently she got diagnosed with OSA, is now a faithful CPAP user keeping her AHI under 1 most of the time. Lately she has had had some blood pressure issues and fluid retention, they would adjusther meds and life goes on.

This time the fluid retention came back despite lasix and other meds. The are now giving her the diagnosis of pulmonary hypertension from the years of the untreated OSA and a complication of the ASD repair. Anyone out there know of how to manage pulmonary hypertension, treatments and so on. They initially thought it was CHF, but the cardiac output is fine and the pulmonary function is fine.

Sure.

Run down to the cardiac cath lab. Inhale some nitric oxide. If it works start nifedipine.

I have heard that the nitric oxide drugs were developed for the management of pulmonary hypertension, before the "off label" uses came "up"

People are also using Viagra believe it or not. I haven't seen the literature documenting its effectiveness, but I know the ICU where I work uses it all the time.

I just spent an hour composing a response. I was not allowed to post it for very questionable rules. Please have her evaluated by a pulmonologist that has extensive training in the management of pulmonary hypertension.

Thanks very much, but with Viagara being a front-line drug for PH, and since it's so easy to get online, I think we're OK now.
