Newbie with S9 and Data Before and After Diagnosis

Post by jemem » Thu Mar 04, 2010 10:57 am

First I would like to thank everyone here for everything that I have learned about OSA and the treatment of OSA.

I went into this with lots of info from the start. My Dr. said I was not his typical patient. I am 32 years old 6"2" 185#. He looked at my throat and said he would not be surprised if I had OSA because my airway is low and small or constricted. Ordered the study.

Initial study "The findings indicate obstructive sleep apnea consisting of apneas and hypopneas with associated arousals and oxygen desaturations which was most prominent in the supine position. Obstructive sleep apnea may be related to other medical conditions. Clinical correlation is advised. The patient will be scheduled for a follow-up visit along with CPAP titration study."
EEG: N1:6.2% N2:69.1% N3:2.8% REM:22%
Respiratory: AHI:14.8 NREM AHI:16.6 REM AHI:8.2 Supine AHI:23.9
Total respiratory events: 98 (95 obstructive)
Arousal index: 19.6
Minimum oxygen saturation: 88%
Snoring: Loud
EKG: Occasional PVC

CPAP titration "The findings indicate that the obstructive sleep apnea was best controlled with CPAP at 7cm. of water....CPAP will be prescribed at 7cm. of water..."
EEG: N1:4.5% N2:58.4% N3:15.5% REM:21.7%
Respiratory: AHI:2.5 NREM AHI:1.9 REM AHI:4.6 Supine AHI:2.5
Total respiratory events: 15 (0 obstructive)
Arousal index: 11.2
Minimum oxygen saturation: 96%
AHI with CPAP at 7cm. of water: 1.2
Snoring: none
EKG: Rare PVC's

Night 1 with S9Auto on straight CPAP mode level 7 and humidifier set on 3.5 using auto climate control and swift fx mask.

Conclusion: I am a happy man and my wife is a happy woman!
I'm not real sure abut the leakrate but I did wake up with my mouth open once. I'll try a chinstrap or different mask if this continues but AHI is good. From what I have read on this forum there will be bumps along the way but I know that there are lots of great members here that will be a great resource while living with and treating OSA.

Sincerely,
JEMEM

Looking good! I am impressed with the detail on the LCD display itself... I have an S9 AutoSet on its way with delivery scheduled for Monday--looking forward to this.

It is impressive that you slept a full 8.6 hours with the mask on your first night--you are one of the lucky ones. I think your leak rate is still considered to be in the acceptable range (thus the smiley face for mask fit), though you have room for improvement.

Special thanks for showing that display -- it's good to know the do give separat AI and AHI data.

Hi together,
it's really a great experience to see how much knowledge is available in this forum. I'm a Newbie in matters of APAP. Last Thursday I got my S9 and I'm now trying to understand what all the collected Data could mean. I hope some of you can understand what I write because my English is more German than what it should be.
My wife told me that during the night I produce some strange noises which sound like air flowing out of my mouth. As a first approach I used sleep aid on my Iphone and could verify the sounds. At this time the flow data look like no.2. During a time without any strange sounds the data look like no.1.
My question is: Could anybody confirm my theory, that a missing negative flow is an indication for sleeping with mouth open? If that’s the case I’ve to talk to my Doc about a full facemask.
Many thanks for your support.

I don't know the answer to your question about flow, but do you also find a spike in your leak chart at the same time? Mouth leaks should show up there if you are losing air that way. Another option instead of a full face mask is a chin strap.

Many thanks for your quick reply.
I think the algorithm of the S9 for leakages is more sensitive to mask leakages than to ‚mouth leakages‘.
As you can see on the third graph a mask leakage seems to decrease positive flow values but not the overall shape of the curve. If that’s the case Resmed could do easily an update to implement a mouth open time counter as well.

JEMEM - great to hear about your experiences. I'm a newbie as well, haven't gotten Rx yet but should this week and then off to the races. Based on the tech summary from my titration study I expect I'll be pretty similar to you (I'm a bit older - 39 - but otherwise very close (6'3" 190#) and expect a setting at 6cms.

Out of curiosity: what path did you take to get the S9? Did you get from DME, and if so what was the Dr's Rx that got it for you? I'm aiming for an S9Auto and SwiftFX as well, so looking for any tips on how to get there.

I had to pay a suplement for the S9 Autoset, but they acccepted my old S8 in lieu of the extra payment. I had to switch DMe's to cut that deal. Worth a try.
