APAP Data Capture

Results from APAP & oximeter (ResMed Autoset Vantage & Respironics 920 Plus) loaners. Please help me understand what these numbers mean -- thanks! I've stubbed in for tonight (Monday)'s sleep.

7/2: SpO2=forgot to write down, Pulse=64
7/3: SpO2=98, Pulse=80, last reading before sleep SpO2=94, Pulse=66
7/4: SpO2=98, Pulse=80, last reading before sleep SpO2=96, Pulse=78
7/5: SpO2=98, Pulse=70, last reading before sleep SpO2=96, Pulse=64
no more recording oximter...I miss it already  :D  

Wakeup:
7/2: SpO2=98, Pulse
7/3: SpO2=96, Pulse=93, Pressure 9.5  must have followed an event, wide awake & breathing very deeply
       Sat up & numbers changed: SpO2=97, Pulse=95, Pressure 9.7 increased from wakeup (8 secs post event?)
       Mask off & changed again: SpO2=96 then 95, Pulse=80 then 78
7/4: SpO2=97, Pulse=70, Pressure 7.7 
       Sat up & numbers changed: SpO2=97, Pulse=84, Pressure 7.8 then back down to 7.7
       Mask off & changed again: SpO2=93, Pulse=88
       8 mins later SpO2=97, Pulse=78
7/5:SpO2 batteries died so no data, Pressure 5.7 which feels like too little air
7:6: 
OptiLife Leak rates: 5 cfm=16.1 L/m (0.27 L/s), 10 cfm=23.1 L/m (0.39 L/m)
“Mask=standard” setting.

Nighttime Arousals/ Exhale Puffs/Wakeup State
7/2: trbl falling & staying asleep, many wakeups & 2 puffs. Tired & irritable but alert & not sleepy.
7/3: fell asleep easily, several arousals, 1 puff. Tired, marginally better than yesterday.
7/4: fell asleep almost immediately, 3 arousals (2 w SpO2=97-97 & pulse= 100+...yikes) including 1 puff. Less tired, more awake & alert.
7/5: fell asleep quickly, 4 arousals (2 puffs, I with SpO2=97 & pulse=103...yikes!)

Although your AHI looks good for both nights, Your leak is too high. For your machine anything over .40 is way too high (try to keep it under .20). The machine already figures in the exhaust rate for the mask, so any leak shown is an actual leak. With such a high leak rate I wouldn't trust the data. You may have had events that weren't recorded. Could you be mouth breathing/leaking?

Keep in mind, Resmed report the Leaks in L/s, not L/M, and keep in mind that the OptiLife is NOT a Resmed mask, therefore the Mask Selection should be set to Standard. And in the Standard Mask Selection the Resmeds SUBTRACT 0.4 L/s (24 L/M) from the reported Leak rate so you must ADD that 0.4 L/s to the reported Leak rate to then compare that figure to the allowed vent rate for the OptiLife and the reported Pressure.

So my leak rate of about 0.4 L/s is really (0.4+0.4) 0.8...yikes! I'll hafta wait for the doc to get the detailed data as the APAP pressure changes because I don't know how to compare the leak rate at teh APAP's varying pressures. One thing's for sure, I've got LEAK . Guess a hybrid style mask is called for 'cause the idea of taping my mouth with my unpredicatible blocked-nose congestion is just too scary.

PS Anyone know how to get the leak rates for ResMed's masks...maybe there's one with similar rates to my OptiLife that I could set.

All masks, every brand and style, come w/literature. And in that literature is a table chart w/allowed vent rates running up the left side and xPAP pressures running along the bottom in 2 cms increments. Where allowed vent rate and reported pressure rate meet is the allowed vent rate at that pressure for that mask. And the figure to be compared to the reported Leak rate plus 24 L/M. (The literature always uses L/M rather than L/s).

I've never quite understood WHY Resmed reports the Leak rate in L/s. Via the software the Leak rate reporting can be changed to L/M. I don't understand why L/M wouldn't be the Default since even Resmed's mask literature's allowed vent rate chart is in L/M.

OK, so it's compare (rpted leak rate+24) to the mask's reported leak rate at specific cfms (ensure all are in l/m). I still don't know if the ResMed APAP reported pressure is an average for the night (or the highest) so not sure which cfm to use in the mask's leak rate chart. At any rate, if I use the reported overnight pressure & leak rate, then:

DATE...Pressure...Leak+24..........Mask Leak
7/2: ....6.9........27.6+24=51.6....5 cfm=16.1, 10 cfm=23.1
7/3: ....9.2........26.4+24=50.4
7/4: ....7.7........24+24=48

This an eye-opener...wow wow wow...no wonder I'm having problems!

Thanks Slinky! I really appreciate walking me thru how to understand the overnight reported leak rate vs the mask's leak rate.

Both reported Pressure and Leak rates are the 95th percentile, i.e. the pressure or leak AT OR BELOW which you spent 95% of the night.
