Well, I don't know what happened last night.. but I've never had numbers look this awful before. My AHI was high when I first started my CPAP therapy, has mellowed out the last few weeks, and is just starting to rise again. Last night I ate dinner early and went to sleep at a decent time. No alcohol, caffeine, or anything!
Oh. While writing this, something dawned on me. I tried a new pillow last night. Could that account for these numbers?

A lot of different things can disrupt sleep; the few you mentioned that were not factors are just some of them. I know this past week I have felt lousy when I woke up and my numbers were just as high as yours. BUT.......what I didn't know was that I was getting sick. I now have a full-blown head cold. That's just another thing that can effect your rest - before you even realize it is happening.
I wouldn't think a new pillow would effect your therapy, especially when your leak rate is so low. It could, though, I suppose, if it put your head/neck in an awkward position which would tend to block your airway....

Perhaps your new pillow is so comfortable that you ended up on your back--when apneas are usually worse--and the machine's pressure wasn't enough to prevent those apneas or hypopneas?
