Help Ultra Mirage or Mirage Quattro????

Post by gw » Thu Jul 10, 2008 11:03 am

Nothing is working with my high pressure (20). I am on a bi-pap and loosing too much air through my mouth even with a chin strap. So I was thinking of trying a full face. Do these cover the chin also and what is the difference between the 2 other than price?

I use a medium wide profile lite, so I am not sure what size full face to get. My nose is more wide than pointy and not much bridge and my face is chubby with not much chin. I know sounds great huh?

Could use some advise and some sleep.

Many people who have high numbers benefit from a FF mask. I have used both the UM and the Quattro FF masks. The Quattro leaks much less for me.

Unlike the UM, the Quattro has a plastic piece that runs all the way around the inside of the cushion. I believe that it helps to stabilize the cushion.

Others have complained about leaks with the Quattro. For us sleep deprived SA patients, sometimes it's difficult to pay attention to detail. I wash the cushion with lukewarm water every morning to get rid of the face oils. That helps it keep a better seal the next night.

When I assemble it at night, I press the cushion onto the frame. Here are two important points that some might miss. Next I _press all around_ the frame (360 degrees) with a finger to be sure that the cushion is firmly inserted in the frame (not sitting up at any point). Then I finish by closely viewing the edge of the frame (where the cushion connects) to see that there is a _straight line_. If it is not inserted correctly, you can see a little "wrinkle" where it's not right.

That happens to me maybe twice a week. If I had not taken the cushion off and started again to fix it, I'm sure that the leak rate would be high.

The other thing that is important (it's been mentioned here before) is to use some teflon tape (for wrapping pipe threads) and wrap the elbow part of the frame where it goes into the body of the frame. This further reduces leaks.

Doing both of the above, I has an avg leak rate of 26 according to EV. I think that's pretty good. More importantly, I don't wake up all night with leaks into my eyes, which I did with the UM.

ResMed makes good masks and both of these are good. I have the UMFF, but had a hard time getting the right one. If you get it from a DME, make absolutely sure they use a series of 3D templates (available from ResMed for n/c to them or to you). A medium leaked too bad and had a lot of trouble getting air blown into my eyes. Fortunately, they exchanged it for a Small, which works great. There is a basic difference in the two masks in terms of how they fit the nose bridge. I believe the Quattro is for a wider nose bridge, but you can call ResMed and they (unlike Respironics) will have someone talk to you about their masks.

The Ultra has slightly lower ratings:

I plan to get one of the two soon. For my very first mask. Leaning toward Quattro, will see what the DME fitting suggests for me personally.

I've used both.

I greatly prefer the Quattro. (My pressure is 13, so relatively high.) Lower profile for side sleeping (and I'm also a stomach sleeper, and manage to make it work with the Quattro, though the mask marks aren't pretty); greater adjustibility, and quieter venting.

On both masks, the bottom of the mask hits the top crease of my chin (just below the lower lip).

I'm still trying to find that "perfect mask" (especially since I have sensitive skin and eyes that get beat up/swollen if I use the FF mask too many nights in a row....I've also got a Hybrid in the rotation, and hope to find a good nasal pillow or prong mask at some point)--but you won't regret having a FF mask in the nightstand for those times when you're congested or just can't get a good seal on a nasal mask.

I too.....have used both, and still alternate between the two, but
do find the Quattro to be more comfortable. For myself, I find the
pressure adjustment on the bridge of the nose for the Quattro to be
a better comfort feature. As far as leak rates are concerned, I've
been pretty dead even, so cant make a judgement there. As someone who
is a chronic mouth breather, cant imagine where'd I'd be with CPAP without
these masks. I used the Mirage during my titration at the sleep lab, but
was provided the option of the Quattro from my DME. I purchased my Mirage backup (that was the plan, as backup) from cpap.com, and find the shallow size small mask fits my face best. So for each mask basically, they have their up-sides, and I would recommend trying both.

I have been using the Quattro for many months and find it a good mask. I've had many nites at 0.00 leak rate with it. I've never tried the Ultra, but the DME has told me that the vent holes are far noisier than the Quattro. the Quattro's vent is pretty much silent. The vent holes are less numerious and designed differntly on the Ultra.

All in all, I think I'll stay with the Quattro. I strap it on fairly good...like a hat that's been on your head for awhile...soon it feels like it isn't there. I tried to let it "float" on my face loosely, which seems to work for some people, but didn't work for me. That allows the double cushion to fill with air and seal. Even with identical masks, it's such a personal adjustment thing. And, there is nothing symetrical about most human faces.

For anyone who does not have a "pointy" nose bridge but a more wide or square type so to speak, the Shallow, if available, in a mask works MUCH better than a Standard size. The Ultra Mirage does have a Shallow. That is why I could not use it in the beginning, I needed a Shallow and did not realize it. Now, I can use the Ultra Mirage FF, with a Shallow cushion and have very few problems with leakage. I can also use the Ultra Mirage 2 now, in Shallow, where as I could not use it before. Also the Shallow in the Activa mask works perfect for me.

I, too, use both, but I prefer the UMFF over my Quattro. Neither extends under the chin. There is a mask that does that--I tried it, didn't like that feeling at all (can't remember which mask that was).

I used the Mirage Quattro and find that with a new cushion versus one that has been used for a couple of months, that it seals well. I have a small, short, narrow face with a narrow bridge of the nose. There seems to be quite a bit of space at the top of the mask that would accommodate a wider bridge - however it doesn't see, with the sharp angles of the mask that it would accommodate your face shape. This is a guess at best on my part.

Perhaps you could see is your DME has one that you can look at in the package.

Might as well go ahead and get BOTH. That way you can be "the decider".

Make sure you get the proper size. CPAP.COM has a mask sizing guide and there are some others, I believe, that can be ordered or downloaded from ResMed's site. The Quattro tends to run just a little bit larger in the same sizes than the UMFF. I think the UMFF might hold up to the higher pressure a bit better than the Quattro......but I've personally never done a test with my Quattro. I had my UMFF up to 18 cm when I started out, but it was not for very long.
