2nd Sleep Study - increased pressures

Hello - my first study was summed up by my Dr. "you had an AHI score of 44 on your side, 59 on your back and 96 during REM sleep (normal is <10, Severe is >30) which places you in the severe obstructive sleep apnea category. Your oxygenation dropped to 80% at its lowest which is also somewhat low (normal is >90%)."

I have since had my AirSense set to 4cm H2O to 8cm H2O. That seems to have helped quite a bit - though I'm still in the catch 22 of sleep meds getting me through the balance of my nights. I haven't yet got the AHI from my second study - He said "Based on the last sleep study. They recommend CPAP therapy at 11-15cm H2O."

Is this a drastic change? I use the dreamwear nasal pillows and am wondering if this increase will be too intense. Thanks in advance for the helpful info. K

Bring up the lower pressure a cm at the time every few days.

Expect some sore chest muscles the first little while as they work at breathing out.

4-20 is very low. Most people feel choked at 4.

My pressure floats between 10 & 11 most nights.

BlackSpinner - you mention changing the pressure incrementally - I have the airsense 10 - those are only changeable remotely or bringing them in to a DME...right? I certainly can't change my own pressure - and I doubt my Dr would allow any flexibility in changing the pressures daily...? Still new to this whole experience - appreciated. K

All machines can have their settings changed manually at home if a person wants to do it. No need to take it in to a DME or have them do it remotely.

See the clinical manual for how to get into that setup menu where things can be changed.

So you could change the pressure if you wanted to. Easy to do.

Hey Pugsy - I have the clinical guide - looking at page 14 - Accessing and exiting the Clinical Menu - before I got too excited about changing everything I tried their first instruction: "Press and hold the dial and the Home button for three seconds.
The Home screen is displayed with an unlock icon in the top right corner of the screen." I tried this many times - absolutely nothing happens. I don't have the elite - is it the case that to enter the clinical settings you must have the elite as mine does not appear to allow it. Many thanks again for your help - - I just realized my profile machine was saying "Elite" so I changed it back to the regular basic model - that must be one of the differences.

You seem to be confused about the machine that you have. You have Autoset listed in your profile, but in your post you say it's the basic model... It's printed on the bottom right of your machine.??????

It doesn't matter which AirSense 10 model machine you have (at least in terms of accessing the clinical set up menu) because they all have the same instructions for entering the clinical setup menu. If it isn't working for you then you must be doing something wrong.

All the AirSense and AirCurve models use the same set of instructions for entering the clinical menu mode.

"AirSense10 - AutoSet" is what it says on bottom lower right of my machine. After my 4th cup of java....oh...Epiphany reached...!

OK - well the irony is not lost on me in my zombie state of brain-deadness that I missed the 2 buttons at once aspect. Ok wiping the egg off of my face and soldiering on. So I was at a 4 - 8, Dr recommends 11-15, so I'm trying 7 - 12 and see how that flies.

Thanks again - this site is amazing. Funny that Apria didn't tell me I could change the machine myself, must be liability laced thinking.

YAY KRL!!!

Red pill or blue pill?
Began XPAP May 2016. Autoset Pressure min. 8 / max 15. Ramp off. ERP set at 2. No humidity. Sleepyhead software installed and being looked at daily, though only beginning to understand the data.

DMEs are part of the medical establishment and they can't go changing the settings on the machine without a doctor okaying the changes. Very similar to a pharmacist not being able to change dosage or medications without the doctor okaying the change.

Now you as a patient aren't bound by those rules. There's no law that says the patient can't change something if they want to and know how.
