Dear Pugsy,
Pugsy wrote:Your model machine is not a very common model to be dispensed. It's usually dispensed because of some sort of special circumstances. Something to talk to your doctor about along with the aerophagia issues (the air in the belly) that you are experiencing.
Do you have any sort of special lung issues going on?
I do not have any special lungs. At least not that I know of. My doc is very nice but keeps telling me not to change the parameters... That that is his job. Concerning the machine he didn't say anything. Just that it is one with ACV.
His first setting was
ASVAuto Min EPAP 4.0 Max IPAP 13.0 PS 2.0-7.0 (cmH2O)
I changed it to
ASVAuto Min EPAP 4.4 Max IPAP 12.4 PS 1.4-6.4 (cmH2O)
ASVAuto Min EPAP 4.4 Max IPAP 13.0 PS 1.0-6.0 (cmH2O)
My Doc now set it to
ASVAuto Min EPAP 4.4 Max IPAP 12.0 PS 0.0-5.0 (cmH2O)
He thinks the aerophagia is not a problem and that it may just go away. Mean while I feel like an airbag each times I wake up at night. I use my bike to go to work and don't really need an airbag.

Just a personal question hope you don't mind.
Paladier thinks DH things you are a deer !
Pesser things you ARE a dear !
Isn't Pugsy a dog ???
Who are you then ? Do you have hoofs like palerider ? and who is that black cat ?
Common sense maybe tough for CPAPER that waited too long before treatment and fried out their brains. No ?
Should I put a warning in my signature ? Esel is Dunkey in german...

There is a dog breed...Pug. I have had more Pugs than I can count over the years and at times more than one Pug.
Pugsy was a name given to me by a friend some years ago at another forum. I liked it and kept it and it sort of defines me.
The deer in my avatar now...they were babies that grew up out here where I live. I watched them grow from tiny twins to big enough to jump the fence and move on...so about 6 months I guess it was. They weren't afraid of me and I could get within about 30 feet of them to watch and take pictures. I have tons of pictures of them.
So I guess I am sort of the "deer" lady and some people change it to "dear" lady.

Aerophagia can just happen and sometimes it will go away and sometimes it won't. Air going to the airway sneaks down the esophagus and into the stomach. Maybe because of a rather lax esophageal sphincter from maybe some years of reflux (or heart burn type of damage). It can happen at any pressure but seems more common at higher pressures.

Your pressures aren't really all that high in the grand scheme of things. Lots of people use higher without the air in the belly issues. I know that isn't much consolation for you though.

If you don't have lung issues....what type of sleep apnea do you have? Any mention of central sleep apnea?

Your machine settings aren't really the settings for central apnea...at least in terms of what we normally see.

Your settings are more in line with what we might see from a bilevel machine for people who are given bilevel maybe just because they have problems exhaling against the pressure OR problems with aerophagia. Often aerophagia issues cause the person to go to bilevel (that's what your ASV machine is) and bilevel gives the needed relief.
Unfortunately these settings still don't offer enough relief from the aerophagia (air in the belly) issues you are having.
Your EPAP is already just about as low as you can go and with PS of 0.0 minimum you aren't going to have much IPAP to start with until apnea events start driving the pressure up and maybe start causing PS to kick in.
Are you spending much time with the pressure maxed out at max IPAP of 12?

I can understand the original poster's comment. For those of us not used to navigating our insurance, it's confusing. I am still confused over Cigna (my insurance) vs Carecentrix vs American Home Patient. Nobody could give me a clear understanding of what my benefits were paying for, and whether I was renting or leasing or purchasing the machine. After a few calls and emails, the best I could figure out is that after 6 months of being compliant I would own the machine. If I wasn't compliant, then the monthly payment for the machine would have been lost.

I'm still not sure. I paid for 6 months, and then the bills stopped so I assume I was compliant and now I own the machine. But they don't make it easy to understand.

Insurance companies generate and profit by your confusion.
