36.6 AHI with CPAP machine

I have a Resmed S10 Autoset. Started using it in April. Seemed to work for a while. Min and Max air pressure was 7 and 14. Over the last month sleep has been getting worse and worse. Waking up not breathing feeling like I'm going insane and walking around the house trying to not go to the ER. Can't function. Last night events per hour was 36.6. Doctor is no help. I changed the min and max values last night to 9 and 16. Didn't help.

What is that AHI made up of?
How much of it is Central Index?
How much of it is Obstructive Apnea Index?
How much of it is Hyponea Index?

What was your AHI at your sleep test? If it was significantly worse, CPAP is still doing "something!"

That being said, this number needs improvement. Do you have stats on mask fit and leakage?

I don't know any details about what makes up the AHI number. The machine just tells me "events per hour." I know the number during my sleep study was in the low 20s. The first few months using the machine the number was around 3-5. I'll download the sleepyhead software and see what I can learn from that. I use the pillows mask and the machine says mask fit is good. About a month ago I stopped a med for anxiety/depression... Wondering if that could be affecting the number. I'm at my ropes end with all this.

So last night was 36.4 AHI. Of those I had 261 clear airway events and three hyponea events. Two large leak events (time over leak redline was 0.094%). 59 obstructive events.

The bulk of your AHI is Clear Airway/Central events.
Did you have a titration study done in a sleep lab where they put the mask on and they figured out what pressure you need....or did they give you an auto machine with the idea that the machine would figure it out?
Does your doctor know about these results? If not, he should.
Do you have a copy of your original sleep study done for diagnostic purposes...does it mention centrals?

I don't know if the OAs are causing arousals which are causing the centrals.
Do you spend much time awake during the night with the mask and machine one?

Perhaps we need to look at the daily detailed graphs to see what the pressure is doing.
You may have a case of complex sleep apnea going on which is when someone has central apneas along with obstructive apneas. If you didn't have very many during the diagnostic sleep study then it's possible that the cpap pressure is causing the centrals.

The sleep study was done back in 2013. I'll look around for the results. My family doctor ordered the CPAP for me in March of this year and honestly I think he just chose what are fairly common numbers for the air pressure... I think.

I don't spend much time awake during the night with the machine on... Little to none. I do get up a couple times during the night to use the bathroom.

You need to be seeing a real sleep doctor about those centrals.

Do you take any medications of any kind? If so, what?
Particularly do you take any medications that might suppress the respiration?

I don't think it is related to higher pressures vs lower pressure...they are ugly when the pressure is low also.

Is this a common pattern where not much happens the first part of the night and then all hell breaks loose the second half?

One thing we can try....turn EPR off. See what happens. Don't change anything else.

Thanks for the help. What is EPR?

I notice breathing problems when first falling asleep and then yes in the last couple hours.

I currently take Seroquel before bed. I was taking Effexor up until the first week of August of this year and then quit.

Injury?
Adding a new medication?
Change dosage?
Recreational "stuff"....you can keep that private but in this day in time I have to ask.

Something changed.

EPR Exhale Pressure Relief...turn it off...have to go to the clinical setup menu.

How long had you been on the Effexor? What dosage and how did you stop taking it...cold turkey or wean down.

When did your start the Seroquel?
