When does it get better? I am using a loner APAP and the provider is monitoring my progress (or lack thereof).
After more than a week of trying the Resmed with nose pillows I don't see any difference the way I feel when I wake up. I still get up 4 or 5 times a night and because of the thing in my nose I cant get back to sleep for a while.
Any advice?

Advise...... don't be so quick to expect results, after all, you have had poor or no sleep for along time. It takes time to retrain your brain to accept new things. I would suggest that you do a lot of reading here on the forum and educate yourself on this therapy. Your brain is telling you it hates this mask and air blowing, change it now Relax.... and read up on what others have experienced, whomever told you that you will recover instantly from years of not getting restorative sleep is an idiot.
As I said RELAX, keep using your mask and cpap, you will get there soon enough.

Sorry, but most of the time CPAP is not like a light switch, boom your better and the world is good. You can get there with time AND tuning the machine to your personal liking. Most people adjust lots of things to get a good nights sleep: pillows, blankets, temperature, mattress, light levels, ear plugs....and now you have an alien sitting on your face blowing air up your nose. Depending on what machine you are using, you should have lots of what are called comfort settings, you need to be comfortable to sleep
Depending on your settings you could also have to have your therapy settings adjusted. PLEASE don't just wait for it to happen. Tell us what equipment your are using so we can help you( even if it's only information that you can talk with your Doctor about).

Way too early in the process to be at your wit's end. My advice is be patient, but don't just sit back and rely on the medical experts to optimize your therapy for you. Take some ownership of the problem. This is a great resource. Start reading everything you can on here. Learn about your problem. Be willing to share some of your information and people on here will help you. This has not, in my experience, been a condition that is magically cured by plugging in a machine. It's a process. Finding a mask that fits comfortably. Finding the humidity settings that work for you. Learning to use the downloadable software to monitor your progress. Educating yourself enough on the condition that you can intelligently discuss it with your doctors and equipment providers. Commit yourself to your treatment and dig in for the long haul.

This therapy is "baby steps". It depends on how long you've had it, how long it will take to get your therapy optimized by tweaking it, how long it takes you to pay off your "sleep debt" (with more, good-quality sleep).
Under that criteria......it can be MONTHS for you to feel some benefits. It's not an instantaneous type of recovery. For most of us, it was a gradual thing.
Stick with it, get the Sleepyhead software to monitor your therapy and it will eventually pay off.

Your health takes time to cure and doing other things to improve your sleep like a diet change.(drinking one less cup of coffee at a certain time might save one arousal time instead of sleeping.)

As Wulfman said, it is usually not an instant thing. After three or four months, I began to notice I could sit and work the crossword puzzle in the paper, watch TV or read and not fall asleep. I continue to have a few arrousals (usually less than two an hour), but this is much better than the 49 or so I was having prior to treatment.

I complained a lot to my doctors and respiratory therapists about not acclimating to the machine. After a year, I still could not love my machine. I even alternated days to make sleep easier. But after not sleeping on cpap for over a week, I noticed a relapse in heart attack like events. This was a wake up call. Although I sleep like crap on the machine, I cannot live without my apap. My apap literally saved me from having frequent arousals that felt like my heart was going bonkers. Not trying to scare you but I wanted to put things in perspective. Many users here often report about better sleep, and I find that discouraged me a lot even though it meant to motivate me. I felt like I was alone in my struggle to get sleep on cpap. But I later discovered there are many cpap users who use the machine, and do not sleep well on it, but it does improve their cognitive function. I think it is better to be able to think and be sleepy, than be both sleepy and feeling like you cant think.
It's not a easy transition but things will get better.
Have you tried different masks? a Hose cozy? Or putting the machine below your bed? Or even a mask liner? A nasal pillow is not always the best mask for everyone. I thought it was but when I switched to full face, I realized breathing became more normal on a full face than with a pillow. There is a noise trade off. But if breathing normally is a struggle, I would try out a minimal contact full face mask

Don't give up. When you become acclimated to the therapy it will be worth it. Just have to stick with it.

Thank you thank you thank you all for those words of encouragement. I gave it one more shot and found that the Resmed Airfit P10 suited my needs. I also went back to my original old Resmed tango set at 9.5 on the advice from my physician.
Nigh time arousals are less frequent and I wake up ready for my treadmill exercise.
