So, I've been on ASV therapy for almost 4 years now and was resigned to the fact that an AHI below 3 was probably as good as it was ever going to get. Recently, I started looking at my data more carefully and began tweaking my settings with the help of SleepyHead and Encore Basic to see if I could do better. Over the past month, I have had nights where I would have one hypopnea in a given session even though a look at the flow diagram really didn't show anything. I was convinced my ASV had it out for me and would always find something to flag so that I would never get to that elusive 0.0. I know, it's just a number, but it's nice to have an objective goal along with the more important subjective goals in this journey. Exhausted last night from work and our 6 month old overcoming a sinus infection, I decided to have a Medicinal Makers Mark Manhattan after dinner to relax, even though it might cause my AHI to go up and possibly compromise my sleep quality. Well, apparently it didn't. Last night I had my first AHI of 0.0 ever (link below to Encore Basic Daily Report) . EPAP pressure was up higher than it usually is most of the night, probably from the consumption of alcohol. However, this may be telling me I might want to think about raising my EPAP min from 6 cm. Most of my untreated hypopneas have been happening at 6-7 cm. I will have to continue to monitor my data closely as one really should not make changes based upon one night.
I guess the moral of my story is this: I was dejected when I was handed an ASV after they found CPAP induced Central Apnea with CSR (CompSA) in my APAP trail and basically told me good luck, we really can't help you from this point on. My sleep doctor has basically been absent or incompetent the entire time I have been on ASV. I have had to titrate and optimize my own therapy from day one. The sleep therapist told me I wouldn't be able to do it, but I have. Most importantly, I feel better and I am living better. So, for those of you on ASV: yeah, it can and will be difficult to tame this beast, especially on your own but with the help of people here on CpapTalk, you will if you stick with it!

Congratulations on attaining that elusive goal. I know what you mean about the machine being out to get you and keep you just a little one event away from 0.0. I have that happen all the time...one or two little somethings to keep that 0.0 elusive.
