My AHI is really low, but I still feel tired a lot

Post by fmj13 » Sat Nov 23, 2013 2:11 pm

My AHI levels are really low, but I still don't feel I am getting much in the way of deep sleep. The last 3 nights, I have been really tired not long after getting home from work and basically feel asleep watching TV a couple of hours before I would normally go to sleep. Today, I got up in the morning tired and ended up taking a 2+ hour nap with my BiPap. What else can I do?

It would really help if you would list what your settings are.
If you're using your "auto" Bi-level with a range of pressures, it's possible that the pressure changes are disrupting your sleep. Maybe you need to try "straight" Bi-level settings......single pressure IPAP and EPAP.

My limited experience with napping in the early evening and then using my CPAP is that I get the same results.
Ealy evening nap leads to very poor rest later.
Try to avoid the patten of falling aleep while not hosed up and in bed.
This might help.

Have you been to the doctor recently and had some blood work done? I used to wonder why I was still tired all the time even though it looked like I was getting good therapy. Fatigue comes with all sorts of issues.

It is a single IPAP / EPAP pressure: 8.6 / 4.8

How do your leak rates look? Could you be losing air through your mouth/lips overnight?

I've also seen posts here about "sleep debt" that has to be "re-paid" before the fatigue goes away. I know I haven't slept well in years. Maybe give it some time?

THere is a lot more to it than the ahi which is the average number of events per hour. It doesn't tell you if you are having clusters of events, how long the events are, etc. My mom had a good ahi, but she was having clusters of events that were longer than 30 seconds each, some over a minute long. She got a new sleep doctor (mine), and he prescribed an auto machine with a range of pressures that could prevent those nasty clusters.

You may need a change in settings. Or you may need a single pressure or smaller range of that is disturbing your sleep. Or maybe an issue with leaks. The graphs from a typical night will help a lot to either spot light a problem or eliminate causes.

Who would have thought it would be this challenging to sleep and breathe at the same time?

How many hours of sleep do you average a night?
Do you have difficulty getting to sleep...staying asleep...if you wake up during the night, how often and do you have trouble going back to sleep?
Do you take any meds...OTC or RX? If so, what, dose and when do you take the dose.
Any other health conditions?
How is bed comfort?
Are you fatigued during the day or sleepy (need to nap really bad) or both? They aren't the same thing. You say you feel "tired" but then comment about the nap...do you find yourself napping lot? If so when, what time and for how long?

If a low AHI was all that is needed to see the miracle...we wouldn't have a need for this forum.
It may just be that you need some time but while "giving it time" it doesn't hurt to take a hard look at other possible culprits.

I usually sleep about 8 hours, but when I wake up (get out of bed) that is usually for 1 hour. I feel like I am aware a lot of the so-called sleep time.
I take fluvoxamine 100mg in the morning.
I don't know of other health conditions, I plan to get more testing by my Primary Physician.
My bed is comfortable.
I usually am not super-fatigued week days so much, but weekends are a nightmare a lot of the times - this has been a constant situation for me. One of my sleep doctors thinks it is "psychological".
This week in particular, I have been really tired on weeknights.

Are you monitoring your therapy with software to actually SEE what's going on during the night?

How's your leak rate? I don't know your history but did a sleep study determine that you needed such low pressure. Your settings seem very low for a bilevel and may be the cause of your symptoms. You may just need more pressure. Of course Robysue uses low pressures too.

All I'll say is that on my brief stint on fluvoxamine, all I wanted to do was sleep. Could that be your problem?
