My new doctor didn't like the unevenness of the 02 line on my pulse ox report and wanted my IPAP increased from 15 to 18. I was able to do that slowly over the course of about two weeks. My IPAP is now 18-23. My EPAP is at 9 (regular AVAPS) which is where it has been for a while.
My comment about sleeping better on regular AVAPS is based on my perceptions. I don't feel I wake up as often and when I do wake, the machine doesn't seem part of the equation. On AE, the FOTs were bothersome, at times, and when awakened, they became part of the equation.
I was hoping that AE would show that it raised my EPAP significantly when sleeping supine, but that wasn't the case. The average EPAP was typically 9.5, which is barely higher than my set EPAP. The last sleep study was pretty clear that an EPAP of 12 was needed to sleep supine. But then again, they weren't using a Trilogy, but the other equipment (can't remember it's name off hand) that can be set to both AVAPS & ASV modes. I suppose there could be a slight discrepancy in how the equipment functions.
Unless there are comments to the contrary, I think my smartest move is to wait until my Trilogy returns then try AE again on that machine. I'll go back through this thread and figure out the starting point. One change at a time.
Until then, regular AVAPS and snoozing in the afternoons. It's still better than before when I slept for HOURS every afternoon. Little cat naps, in my recliner, aren't the end of the world.

I didn't want people to think I'd dropped the ball on this, so here's the latest.
I got my Trilogy back. I went through the settings & options to see what's new, different, etc. I didn't see much that was obviously changed. A new option under a couple of the modes - MPV - which a little research stands for "Mouth Piece Ventilation" which is something I don't need at this stage. Beyond that, I don't *see* much change from the loaner I had.
I plan to give AVAPS-AE another try on THIS machine. My doctor has okay'd my redoing the test, just to see *IF* the updated Trilogy does better or behaves differently than the loaner machine.
If the results are about the same, we feel that the best option would be to increase my set EPAP to 10 on regular AVAPS. I currently have it set to 9, but the AE testing was showing an average EPAP around 9.5. If I can get myself to tolerate EPAP of 10, I *might* be on target for better settings, better sleep and better stamina the next day.
My doctor wrote an official prescription to increase my EPAP to 10 (we need to keep the official records accurate).
I'll start the AVAPS-AE testing again soon and see where it goes. I got my flu shot last week and had flu-like symptoms for several days afterwards. Now, I seem to have pulled/aggravated muscles in my upper back on my left side, causing horrible spasms and pain in my back and chest. I have been MISERABLE and not up to doing anything other than just trying to find a comfortable position to sleep.
Once this has passed and feel more like myself, I'll be trying AVAPS-AE again.

Not sure if the user is still about, but read this OLD thread. The oscilations are indeed expected (almost always 10 in a row). Its the way the trilogy detects if there is an obstruction or limitation or not. It always tests to see if it can improve things by increasing the EPAP, test again, and if no improvement then drop the EPAP, test again, then drop the epap, test again, if it's worse...increase the EPAP...test again... every minute or so.

Oh yeah, I'm still here. I gave up on AVAPS-AE a while ago, using straight AVAPS with the EPAP at a set 10. If pain forces me on my back again, I may try AVAPS-AE again, but for now, I'm okay with the way it is. I wake up every 1.5 - 2 hours, probably because of pain, but can usually move around and go back to sleep.

I am having problems with AVAPS-AE; I am still having oxygen desaturations - presumably during REM sleep. I keep waking up once the EPAP ramps up - I can barely breathe when it's at 9 or 10. I also wonder about the auto-breath rate. In watching the screen it does seem to select a nice, long inspiration time so maybe it's helpful but I'm not sure.
I am wondering - should I raise the PS minimum and shorten the EPAP range? All my sleep studies show I suffer from hypoventilation and thus all recorded sleep events are obstructive hypopneas - no discrete apneas.
In straight bi-level, you are supposed to raise the IPAP for hypopneas and EPAP for apneas. I used IPAP 20/EPAP 4 before switching over to AVAPS with the Trilogy. With AVAPS, I assume I am having desaturations because the IPAP is low and the EPAP has to kick in extra hard in order to prevent/fix the hypopneas. I am meeting my target tidal volume each night.
My settings are:
AVAPS-AE
Min PS: 6
Max PS: 16
EPAP: 4-14
Breath Rate: Auto
I am going to try:
Min PS: 12
Max PS: 20
EPAP: 4-10

Not around much, but saw this and got your PM. Sent you a PM. I stopped AVAPS-AE a long time ago because of the FOT's. Early in my treatment when I was having desaturations, my doctor raised my IPAP. That's about all I can tell you.
