Most people cannot get enough air with only 4. That seems really odd to set it at just 4. It may be why you feel so bad. I would try at least 6.
Also 49 times an hour is not mild. Snubbing above 30 is considered severe.

Sleepjunky, I have found that by using a neck collar, I can use a pressure of 5 and have a really good AHI on most days. I still haven't slept through the night on the machine but it seems due to my highly sensitive body that this is my best shot.
So if you are experiencing alot of apneas, you might want to try wearing one and see what happens. And like others have commented, I can't even breath at a setting of 4. I needed to go to 5 to not feel like I was suffocating.

Kidding aside, positional therapy seems like a good suggestion.
I haven't even got the bill for mine yet. I'll probably feel sick too.
